Alexander Ulintz, MD is an Assistant Professor of Emergency Medicine at The Ohio State University College of Medicine.  In addition to practicing as an emergency medicine physician, he has fellowship training in clinical and health services research.  His primary interest is evaluating how emergency medical services (EMS) clinicians and agencies can impact public health outcomes through new models of care, including community paramedicine and mobile integrated health care. Through his research in post-overdose harm reduction & treatment, he seeks to understand how EMS bridges the gap between health systems and communities.  He serves as Vice Chair of the National Association of EMS Physicians’ Mobile Integrated Healthcare committee and Section Councilor of the American Public Health Association’s Injury Control & Emergency Health Services Section.

Alicia Dinkeldein-Warren has been an EMT with Indianapolis EMS for 15 years.  During her career she has held positions as Logistics Specialist, Grant Manager, Public Relations Coordinator, and currently is the MIH Coordinator.  Alicia has been assigned to the IEMS MIH team since 2020.  She holds a Bachelor of Science in Criminal Justice with a focus in Public Safety Management from Indiana University and will begin work on her MSW degree in 2026.  Alicia has been recognized locally and nationally for work with the unhoused population in Indianapolis.

Allie Turner is a nursing leader with extensive experience in virtual and hybrid acute-care delivery models, serving as an Enterprise Subject Matter Expert and Nursing Clinical Coordinator for Advocate Health’s Hospital at Home (H@H) program. With a background in critical care and med-surg nursing, Allie has become a key driver in expanding innovative care models that bridge inpatient-level services with home-based, community-embedded support.

In her current role, Allie is dedicated to team development and patient-centered care, working toward enterprise-wide operational alignment of virtual nursing, navigator teams, and Mobile Integrated Health (MIH) collaborations across multiple markets. Over the past three years, she has collaborated closely with  partners across the care continuum to expand H@H services throughout the Greater Charlotte and Wake markets in North Carolina, as well as into the Midwest region—strengthening continuity, scalability, and innovation in patient care.

Allie is a member of the American Nurses Association and holds a Stroke Certified Registered Nurse certification. Outside of work, she is currently pursuing her Master of Science in Nursing Administration, continuing her commitment to supporting nursing teams and advancing innovative care delivery models.

Dr. Ameen Jamali is a board-certified emergency medicine physician and the medical director for Acadian Health and Safety Management Systems, where he oversees the development, implementation, and optimization of Mobile Integrated Healthcare programs. He also provides medical direction and oversight for remote industrial medical operations, ensuring quality medical care in complex and austere environments.

Dr. Jamali holds faculty appointment as an assistant professor of emergency medicine at the Johns Hopkins School of Medicine as well as the University of Texas MD Anderson Cancer Center. For more than a decade, he has held tactical and operational medicine roles with federal agencies including the U.S. Secret Service, ATF, and the U.S. Marshals Service’s Special Operations Group. His work has included medical support for National Special Security Events, high-risk tactical operations, as well as domestic and international protective missions of former US Presidents.

Dr. Jamali’s career reflects a commitment to advancing innovative care delivery and operational preparedness.

Amy Schiotz is the Health System Senior Enterprise Executive at CoachCare, where she partners with healthcare organizations to shape and scale strategies across the patient care continuum—from chronic disease management and post-acute transitions to technology-enabled care in the home.

With more than 20 years of experience in patient monitoring, diagnostics, and digital health, Amy has held leadership roles at Masimo, Mayo Clinic, Microsoft, and Best Buy Health. She began her career as a respiratory therapist in Level I trauma centers and intensive care units, grounding her work in firsthand clinical experience and an outcomes-driven approach to healthcare innovation.

Amy holds an MBA in Healthcare Administration from the University of Southern Indiana and is an active member of the American College of Healthcare Executives and the Healthcare Financial Management Association.

Andrew Tinsley is a healthcare operator turned product leader and consultant focused on mobile integrated health, community paramedicine, and tech-enabled care at home. He began his career as a paramedic and has since built and scaled innovative healthcare technology companies from concept to national reach—bridging the gap between frontline care delivery and the digital infrastructure that supports it.

As Founder of Tinsley & Partners, a boutique consultancy, Andrew helps provider organizations and early-stage ventures design, launch, and operationalize MIH/CP programs, Hospital at Home models, and translate operational change into measurable value under value-based contracts. Andrew’s work centers on pragmatic system design: clear triage and referral pathways, right-sized scope for paramedicine teams, data pipelines that don’t break in the wild, and analytics that tie interventions to clinical and financial outcomes.

He developed the “IQ Model,” a proprietary framework that connects clinical and operational activities to economic impact to drive contracting and continuous improvement. Starting her EMS career in 2008, Anna, has provided direct clinical care and education in various EMS specialties including community paramedicine, rotor-wing and ground based critical care transport and ski patrol. She currently works as an Advanced Care Paramedic Specialist providing clinical education, professional development and program development for the organization's Mobile Integrated Healthcare and Critical Care Transport teams. She is the current chair of the CAMTS MIH standards committee. Additionally she holds a Bachelors of Science degree from Point Park University (Pittsburgh, PA) in Public Administration and Emergency Services Management.

I am the Chief Partner-Client Officer for Beyond Lucid Technologies. I have been a Paramedic for over 32 years and worked in EMS for over 33.  During my career I have spent time in both the role of provider and management in both small rural systems to time with New York City EMS and Magen David Adom in Israel.

I have been involved in many programs in the area of MCI preparedness and response; serving as the Planning Section Chief for CT region 3 Incident Management Team deployed with the team in that role to many incidents including the Clean Energy Plant Explosion and Sandy Hook Shooting to name a few.  I also served as a member of the Integrated EMS Response to Active Shooter work group as part of the Connecticut Capital Region. I was awarded the CT Public Health Commissioner’s Award for Individual Impact on EMS in the State of CT.  I have been the author of many EMS related articles as well.

Ashley Koch is the Vice President of Customer Experience at instED where she leads Customer Account Management, Marketing and Sales Enablement. Her team partners with payors, care management teams and network providers to connect patients to in-home acute care services and monitor impacts to cost of care and health outcomes resulting from instED interventions. She has 15 years of communications, marketing, and customer engagement experience. She has held roles in academia and nonprofit media relations before spending over a decade in managed health care, where she led creative development and member engagement strategy for Elevance Health’s government business. Ashley holds a B.A. in Philosophy from Old Dominion University and lives in Norfolk, VA.

Benjamin A. Swig is a Nationally Registered Paramedic, public health researcher, and health technology executive whose work spans public health, emergency medical services, digital health, and AI-enabled innovation. He currently serves as Chief Growth Officer at Sonavi Labs, where he leads product development and global strategy for AI-supported respiratory monitoring, and is completing his Doctor of Public Health (DrPH) at Tulane University Celia Scott Weatherhead School of Public Health & Tropical Medicine.

Ben’s drone-enabled EMS and public health research focuses on public perception and of how unmanned aircraft systems can deliver time-critical medications and devices—such as AEDs, naloxone, epinephrine, and inhalers—during the critical minutes before ambulance arrival and layperson self-efficacy in utilizing delivered medication and devices.

Ben has worked and volunteered in EMS since 2001. He co-founded, Ready Responders, now Mylaurel and more recently Acadian Health. His field experience includes municipal and for-profit EMS, mountain and urban SAR, mobile integrated healthcare, and EMS system development across multiple states and Africa. His research and leadership center on deploying practical, scalable, and ethically grounded innovations that improve access, outcomes, and community resilience.

Brian Olive serves as a Community Paramedic and Logistics Employee Engagement Team (Logistics EET) Lead within the Lee County Department of Public Safety, Division of Emergency Medical Services. Since joining Lee County EMS in 2004, Brian has built a career marked by excellence in patient care, mentorship, and operational leadership.

Beginning his career as an Emergency Medical Technician, Brian advanced to the role of Paramedic and later Field Training Officer, where he played a key role in developing and mentoring new EMS providers. In his current position, Brian divides his time between clinical and operational initiatives, serving as a member of the Community Health Program and leading the Logistics EET. As the Logistics EET Lead, he manages the group responsible for evaluating, vetting, and procuring all medical equipment and supplies used across the ambulance fleet, ensuring operational readiness and consistency in patient care delivery.

Driven by a passion for progress and service, Brian continues to strengthen the Community Health Program’s commitment to patient-centered innovation and supports the larger mission of Lee County EMS in delivering exceptional care to every resident of Lee County.

Dr. Brock Daniels completed his M.D. degree at Northwestern University Feinberg School of Medicine in 2010, and residency training in Emergency Medicine at Yale-New Haven Hospital in 2014. Additionally, Dr. Daniels completed a postdoctoral fellowship in Quality, Safety and Comparative Effectiveness Research with a focus in health informatics at the University of California, Davis Center for Healthcare Policy and Research in 2017. He is currently an Assistant Professor of Emergency Medicine at Weill Cornell Medicine, and Medical Director of the NewYork Presbyterian (NYP) Community Tele-Paramedicine (CTP) program. CTP is a mobile integrated health program that combine community paramedic home visits with telehealth encounters by emergency physicians and proactive care coordination by dedicated nurse care managers. CTP has conducted over 6000 home visits for many of NYP’s most medically complex and socially vulnerable patient populations living with chronic illness. Dr. Daniels has led several MIH-focused research studies funded by NYP, the Emergency Medicine Foundation, National Institute on Aging and the Patient Centered Outcomes Research Institute including a randomized trial of MIH among 2,000 patients with heart failure recruited from 11 hospitals across New York City.

Christopher Goenner is a seasoned public safety professional with over 33 years of experience spanning fire services, law enforcement, and Emergency Medical Services (EMS). Originally from Pennsylvania, Chris began his career as a volunteer firefighter, EMT, and law enforcement officer, serving a decade in full-time law enforcement before transitioning fully into EMS. After relocating to North Carolina in 2007, he earned both state and national paramedic certifications and pursued advanced education, completing an AAS in EMS, a BS in Emergency and Disaster Management, and an MS in Health Sciences with a concentration in EMS Education.

Chris has served as a field training officer at Cabarrus County EMS and held academic leadership roles, including Department Chair of Emergency Medical Sciences at Central Piedmont Community College. He currently serves as Program Chair and Associate Professor for the Bachelor of Science in Community Paramedicine at Cabarrus College of Health Sciences. A frequent presenter at EMS conferences nationwide, Chris has been recognized for his award-winning research on End-Tidal CO₂ in septic patients. He is the past president of the North Carolina EMS Educators Association and continues to serve on multiple committees with the NC Office of EMS.

Constantinos (Taki) Michaelidis, MD, MBA, MS is an internal medicine physician by training and practices as a virtual home hospitalist in the UMass Memorial Medical Center (UMMMC) Hospital at Home program where he is Medical Director and faculty in the Program in Digital Medicine at the UMass Chan Medical School. Prior to joining UMMMC, he was Senior Medical Director in the Atrius Health Hospital at Home program. There, he supported local and national hospital at home program development and implemented new acute (ED in Home) and post-acute (SNF at Home) home-based care models for a predominantly frail, medically complex and home-bound population in Massachusetts.

Courtney Nelson serves as Associate Vice President for Enterprise Ambulatory Care Coordination and Management at Advocate Health, overseeing both the Primary Care Connect and Medicare Wellness Visit Nurse teams. She directs initiatives aimed at delivering comprehensive patient care, enhancing access to services, and advancing preventive health programs. Through collaboration with Mobile Integrated Health (MIH), her teams have achieved reductions in healthcare costs, improved health outcomes, and elevated patient satisfaction by expanding options for in-home care delivery.

Since joining the Primary Care Connect team in 2021, Courtney has broadened support for outpatient clinics across the NC/GA State Divisions. In 2023, she assumed leadership of the Medicare Wellness Visit Nurse team and is actively working to expand this program throughout the Southeast. With over ten years of experience as a nursing leader within Atrium Health's medical group, she possesses deep expertise in family medicine, internal medicine, and pediatrics. Her previous positions include regional patient safety coordinator and clinical supervisor for the high-risk postpartum unit at Atrium Health Carolinas Medical Center. Courtney recently earned her Doctor of Nursing Practice (DNP) in Executive Healthcare Leadership from the University of South Carolina and holds board certification in Ambulatory Care

Colin Johnson serves as a Deputy Chief in the Office of the Medical Director within the Lee County Department of Public Safety, Division of Emergency Medical Services. With over 15 years of experience in EMS, Colin's role encompasses a diverse range of responsibilities including clinical quality assurance, improvement initiatives, patient safety, protocol development, procurement of medical supplies, educational initiatives, and the identification of clinical best practices. Prior to assuming his current position, Colin held various roles within Lee County Emergency Medical Services, including Division Chief, Captain, Lieutenant, Field Training Officer, Paramedic, and EMT. His educational background includes a Master of Science Degree in Psychology and a Bachelor of Science Degree in Health Studies. 

Darlene is the President and Founder of the Danny’s Dose Alliance, an organization dedicated to improving and ensuring proper emergency care for individuals with rare and chronic conditions requiring specialized treatment.

 Her advocacy began 14 years ago following an unexpected family diagnosis of hemophilia, which inspired her deep involvement and leadership within the rare and chronic disease community. Through this work, Darlene has gained valuable insight into the challenges families face; limited access to appropriate care, misinformation, and a lack of education, along with the need for greater awareness of available federal, state, and local resources.

 Darlene’s commitment to improving emergency care has led to advisory roles with EMS for Children and the Pediatric Pandemic Network (PPN) plus others. Partnering with state and national EMS associations, she organized the first Rare and Chronic Care Taskforce, dedicated to developing nationally recognized, disease-specific Emergency Treatment Guidelines.

Her leadership has resulted in EMS legislation in Missouri, Minnesota, and Arkansas, as well as Scope of Practice updates in 17 additional states with ongoing advocacy toward achieving these improvements nationwide. Darlene works tirelessly to protect, support, educate, and empower emergency personnel, patients, and families across the United States.

Dr. Eric Yazel graduated from Indiana University with a B.S. in Biology, followed by a M.A. in Clinical Physiology from Ball State University. He received his Medical Degree from the University of Louisville and completed his residency in Emergency Medicine there as well, serving as Chief Resident. Over the years he has been medical director for numerous EMS services in the Kentuckiana area, both urban and rural in nature. He currently serves as the EMS Medical Director for the State of Indiana. In addition, he also holds positions as an Assistant Professor of Emergency Medicine at the University of Louisville, Attending Physician at Norton Clark Emergency Department, Clark County Health Officer, Chief Medical Officer for LifeSpring Health Systems, and Chief Tactical Physician for the Jeffersonville Police Department.

Eric was born in northern Indiana and grew up in Indianapolis. He has been a Clark County resident since 2006. He enjoys travelling, basketball, and all things outdoors, especially spending time on his farm in Borden with his wife and two children.

Dr. Penni Eggers is an accomplished EMS leader, educator, and paramedic with over two decades of experience across clinical practice, higher education, and fire-based EMS operations. She currently serves as Director of Education and Assistant Professor at the Florida Center for EMS, where she develops statewide EMS training curricula, including MIH education, and supports new program accreditation, and advances workforce development and resiliency initiatives.

Her prior roles include Simulation Manager for HCA West Central Florida, EMS Educator for Palm Beach Gardens Fire Rescue, and EMS Coordinator at St. Petersburg College. She also served as a Firefighter/Paramedic for more than 15 years and Executive Director of the West Central Florida CISM team for two decades.

Dr. Eggers holds an Ed.D. in Educational Leadership from Saint Leo University, a Master’s in Instructional Design and Technology from the University of Tampa, and a bachelor’s in psychology from Saint Leo University. She is a State of Florida Paramedic, Certified Healthcare Simulation Educator, and EMS Instructor, with numerous advanced certifications including ACLS, PHTLS, and EPC.

Her career reflects a lifelong dedication to patient safety, EMS education, and building sustainable, resilient EMS systems that prepare the next generation of providers and leaders.

Diana Libby-Billings is a triple board certified paramedic who has worked in the field for the last twenty years. With experience in 911, municipal, hospital based, critical care transport, and flight medicine; she began work as a mobile integrated health provider for CityBlock Health during the pandemic and hasn’t looked back. 

Passionate about holistic health, LGBTQ+ populations, sexual health, trauma informed care, substance use disorder and behavioral health treatment, she believes that home healthcare is the future of patient care. 

In her off time, she enjoys travel, spending time with her five kids, skiing, sailing, yoga, rebuilding her 1971 VW Bay Window Camper, southeast Asian food, and beginning hobbies that she becomes marginally proficient at until she picks up the next one.

Emanuel “Manny” Mill is a seasoned Community Paramedic and Field Training Officer with Atrium Health’s Mobile Integrated Health (MIH) program. With over 25 years of experience in emergency medical services, Manny has served in diverse settings from frontline emergency response and trauma care to community based paramedicine and clinical education. His leadership extends across prehospital operations, clinical instruction, and special operations medicine, where he has trained paramedics, nurses, medical students, residents, and federal agents.

A former firefighter/paramedic and U.S. Army Reservist combat medic, Manny brings a deep commitment to servant leadership, patient advocacy, and high reliability healthcare systems. His passion for education and compassionate care drives his mission to “love your neighbor one patient at a time,” aligning clinical excellence with empathy and human connection.

Outside of EMS, Manny has been married to his bride for 25 years, is the proud father of three boys, and enjoys life’s adventures with his family and their French Bulldog, Loki.

Dr. Evan Berg is the Chief Medical Officer and Chief Strategy Officer for instED. He is responsible for the oversight of instED’s care delivery model and the execution of the company's strategic initiatives.

Dr. Berg has a significant background in mobile integrated healthcare (MIH). Previously, he served as the Chief of Emergency Medicine at Medically Home, where he developed MIH-based programs to deliver acute care services to patients with complex health issues and an MIH-oriented educational program focused on the assessment, diagnostic and therapeutic skills required in the model. Before that, he was the Vice Chair of Clinical Operations in the Department of Emergency Medicine at Boston Medical Center and an Assistant Professor at the Boston University School of Medicine. There, he managed clinical operations in the adult Emergency Department and fast-track areas, collaborating with instED to reduce hospital admissions through in-home MIH services.

Dr. Berg loves taking care of patients, collaborating with colleagues, and thinking creatively about complex and challenging healthcare problems.

Gerald Root is a dedicated member of the Chicago Fire Department with over 27 years of service. He began his EMS career in May 1991 and quickly established himself as a skilled and compassionate paramedic. Throughout his career, Gerald has held various positions, ultimately earning the rank of Ambulance Commander. In 2021, he expanded his expertise by becoming a Community Paramedic, further enhancing his ability to serve the community.

Currently, Gerald serves as an Ambulance Commander in the Chicago Fire Department's Mobile Integrated Healthcare Unit (MIHU), where he plays a pivotal role in delivering advanced pre-hospital care and addressing public health needs beyond emergency response. His commitment to the well-being of others, coupled with his extensive experience, has made him a respected leader within the department.

Gerald’s career reflects his passion for providing quality care, his continuous professional development, and his unwavering dedication to the communities he serves.

Henry Lewis began his career in emergency services with a small, volunteer fire department in South Carolina in 1999. He quickly transitioned to the local EMS agency and gained his paramedic license in 2003, which he still maintains. As a career EMS professional, Henry has worked across all aspects of EMS
to include governmental, private industry, hospital-based services, and EMS education. He served as the State EMS Coordinator for the state EMS office and in administration for the state’s largest 911 provider before transitioning to become the South Carolina EMS Association’s first Executive Director in 2021. As
a working student, Henry earned his BA in Emergency Management and Masters in Healthcare Administration from Columbia College with honors. He serves as the Chair of the Affiliate Council for the National Association of EMTs and the State Associations liaison to the Board of Directors for the American Ambulance Association.

Dr. Ila Mapp has dedicated 29 years to advancing the nursing profession through roles in travel nursing, bedside care, and leadership. After seven years as a travel nurse, she joined UNC Health in 2008, progressing from clinical nurse to Nurse Manager. Since 2013, Dr. Mapp has led critical care teams at UNC Medical Center, overseeing Rapid Response, ICU Transport, Venous Access, and CVAD Liaison services.
 
Today, she serves as Director of Advanced Care at Home, an innovative program that leverages technology to deliver acute care in patients’ homes. This model fosters collaboration among internal teams and community partners—including paramedics and home health nurses—united by a shared commitment to patient-centered care across North Carolina’s Triangle region.
 

Mrs. Ingrid J. Phillips is a distinguished healthcare leader and a retired Senior Chief 
Hospital Corpsman of the United States Navy. Originally from Yonkers, New York, she 
served on numerous warships and distinguished herself in emergency medical services,
instructional roles, and recruitment efforts. Notably, she was recognized as the U.S. Navy 
Fleet Forces’ first female Sea Sailor of the Year in 2009 and was meritoriously promoted to 
Chief Petty Officer. After retiring in 2017, Mrs. Phillips continued her public service as an 
Emergency Management Analyst and Instructor for Navy Installations Command. Later, 
she joined the Chesapeake Fire Department, where she launched the city’s first Mobile 
Integrated Healthcare (MIH) program, increasing access to healthcare for thousands of 
residents. In 2025, she founded Integrated Healthcare Solutions, offering strategic 
consulting to Fire, EMS, and Law Enforcement agencies on developing communitycentered MIH-CP programs. She also serves as a Strategic Advisor for Julota, supporting 
nationwide care-coordination initiatives. Mrs. Phillips holds a Master’s degree in Public 
Administration, a Bachelor of Science in Emergency Management, and an Associate’s 
degree in Paramedic Studies. She is a graduate of the Navy Senior Enlisted Academy and is 
currently pursuing a Doctor of Health Sciences degree at Eastern Virginia Medical School

• Jamie Pafford-Gresham is a nationally recognized leader in emergency medical services and the CEO of Pafford Medical Services, one of the largest privately owned ambulance providers in the country. With more than five decades of family legacy behind the organization, Jamie has worked in every department of the company, giving her a uniquely comprehensive understanding of the EMS system from the ground up.

  Today, she oversees operations across five states, supporting paramedics serving more than 100 communities, while safeguarding organizational financial strength and evaluating strategic growth opportunities.

  A passionate advocate for Rural EMS, Jamie is deeply engaged in legislative efforts on Capitol Hill and throughout the South. Her work has helped advance reimbursement programs, Upper Payment Limit models, and balance-billing legislation in Arkansas, Louisiana, Mississippi, and Oklahoma.

  Jamie also serves as President of the American Ambulance Association, sits on multiple state ambulance boards, and holds a governor-appointed seat on the Arkansas 911 Board. In these roles, she collaborates closely with hospitals, payors, and policymakers to advance mobile integrated healthcare, improve coordination for high-utilizer populations, and build resilient community-based care models

Jamie Wilson is a recognized innovator in Mobile Integrated Healthcare, combining more than 25 years of experience in Emergency Medical Services with over 13 years as a Registered Nurse to address persistent gaps in healthcare delivery. Through the design and execution of multiple Mobile Integrated Healthcare programs, he has developed scalable, best-practice “last-mile” care delivery models that improve access, enhance care coordination, and drive measurable improvements in patient outcomes.

Drawing on his unique perspective as a Paramedic, Registered Nurse, and healthcare executive, Jamie has led the development of proprietary, collaborative care models in partnership with Managed Care Organizations (MCOs), Accountable Care Organizations (ACOs), and health systems across multiple states. His work consistently bridges clinical excellence with operational innovation, enabling health systems and payers to deliver higher-value care in the home and community setting.

James McLaughlin, affectionately known as “Tiny,” has worked in emergency medicine since 1991 and has served the citizens of Ute Pass Regional Health Service District since 2008. He holds a Bachelor of Science in Business Administration from Colorado Tech, with a focus on International Finance. As Director of Community Paramedicine, James leads the Paramedic Advanced Care Team (PACT), which leverages underutilized resources to serve marginalized and underserved populations. He is trained in Trauma-Informed Care, Motivational Interviewing, and Crisis Intervention, and is a certified Level III Crisis and Hostage Negotiator. In 2021, he was named Colorado State Community Paramedic of the Year. James also chairs the EMS Advisory Committee at Pikes Peak State College, serves on the Colorado Medical Board, and is a Board Member of the Teller County Mental Health Alliance

Jason Fair is the Director of Clinical Services at TANDEM365, where he leads multidisciplinary teams delivering innovative Mobile Integrated Health (MIH) solutions across Michigan. He has held leadership roles in three distinct MIH organizations—TANDEM365, Medically Home, and Emergent Health Partners—each representing a unique model of integrating acute care, EMS, and community-based services. Jason brings a rare perspective shaped by this diversity, focusing on building resilient teams, challenging broken healthcare norms, and designing systems that empower clinicians to lead with purpose.

Dr. Jason Zeller is a graduate of the UNM EMS fellowship and is a board certified EMS physician - previously serving as medical director for a number of first response, transport and flight agencies in Cleveland, Ohio. Dr. Zeller serves as the medical director and EMS physician for Eagle County Paramedic Services, including the organization's MIH programs

Jen Cline is the AVP for Hospital at Home at Atrium Health, where she leads innovative care models that expand patient access and improve healthcare delivery. With 35 years of clinical experience spanning acute care, home health, and inpatient rehabilitation, Jen has dedicated her career to evidence-based practice, patient advocacy, and lifelong learning. 

Previously, she served as the Director of Therapy at Carolinas Medical Center (CMC) for eight years and spent time in academia at Georgia State University. A champion of process improvement and visionary initiatives, Jen has spearheaded programs that enhance patient care and operational efficiency. Her leadership has driven the integration of exercise physiologists into the hospital setting, the launch of an OT critical care residency, and the advancement of interdisciplinary collaboration to improve patient outcomes. 

Guided by Atrium Health’s mission of hope, health, and healing for all, Jen is deeply committed to ensuring equitable care for vulnerable populations. She actively advocates for patients who are unfunded, unhoused, or undocumented, creating opportunities for them to receive essential healthcare. Her passion for service extends beyond the hospital, including work with the HOPE program, pro bono efforts at Samaritan House, and championing professional service projects in Guatemala. 

As a doctoral-trained leader, Jen believes in the power of evidence-based practice and lifelong learning. Her research and dissertation focused on predicting discharge destinations after hospitalization. During the COVID-19 pandemic, she served on incident command and later published a paper in the Physical Therapy Journal of Leadership, Health Policy, and Administration detailing the therapy department’s emergency response. 

Mentorship remains a cornerstone of Jen’s leadership philosophy. She finds great fulfillment in developing future leaders and guiding them toward their highest potential through servant leadership. Jennifer Vigodsky has been employed with the Spartanburg Regional Healthcare System for over 10 years. After working on an inpatient step-down unit, Jennifer transitioned to the Heart Failure Clinic where she became the first Heart Failure Nurse Navigator for the hospital system to assist in reducing readmissions and EC visits. As the Heart Failure Nurse Navigator, Jennifer was able to truly understand the barriers patients face with following up on an outpatient basis. Jennifer has assisted in writing two grants and obtaining funding to launch the Mobile Integrated Health-Community Paramedicine program for the Heart Failure Clinic through the Duke Endowment and the South Carolina Center for Rural and Primary. Since 2022, Jennifer assists in managing five heart failure clinics for the Spartanburg Regional Healthcare System in three counties and reducing EC and hospital admissions for heart failure patients enrolled in community paramedicine by 90%.  Jennifer continues to focus on Social Determinants Health in rural areas

As Senior Director, Jeremy leads Acadian Health operations as the company continues to expand into new markets across the United States.  With roots as a paramedic and more than two decades in healthcare management, Jeremy brings a wealth of experience in leadership, team building, and operational excellence. Prior to joining Acadian Health, Jeremy held key leadership positions, including Senior Director of Mobile Integrated Health and Community Paramedicine, as well as Vice President of Category Management and Contracting for a national hospital at home enablement organization.
 

Jodi Plimpton-Werstein is a Paramedic and Field Training Officer with the Lee County Department of Public Safety, Division of Emergency Medical Services, where she has proudly served since 2006. With nearly two decades of experience in emergency medical services, Jodi has served in multiple roles, including EMT, Paramedic, Field Training Officer, and now Community Paramedic with the Community Health Program.  Jodi played an instrumental role in helping to design, build, and launch the Community Health Program, an innovative initiative aimed at improving patient outcomes through proactive outreach and collaboration. Her work focuses on connecting patients to community resources, promoting continuity of care, and reducing preventable emergency calls and hospitalizations.  In addition to her field experience, Jodi has shared her expertise as an EMT Instructor at the Southwest Florida Public Safety Academy, where she mentored and inspired future EMS professionals. She holds an Associate Degree in Emergency Medical Services Technology and remains deeply committed to advancing community paramedicine through education, innovation, and compassionate patient care.

Jon Johnson, MBA, BSN, RN, NRP, CFRN, TCRN, CMTE, has over three decades of experience serving the EMS community. He holds a Paramedic Diploma, a Bachelor of Science in Nursing, and a Master’s in Business Administration. Jon maintains board certifications as a Flight Nurse and a Trauma Nurse and is also certified as a Medical Transport Executive. In his current role as the Director of In-Home Clinical Services with a national technology and logistics organization focused on enabling healthcare systems to create hospital-at-home programs, he empowers EMS clinicians working with Mobile Integrated Health teams to expand their scope of practice and play an integral role in multidisciplinary healthcare teams.

Previously, Jon served as the Trauma Outreach Coordinator for a large pediatric healthcare system with multiple trauma centers. He also has extensive experience as a Flight Nurse on both fixed-wing and rotor-wing platforms. As an author and contributor, Jon has played a key role in developing educational resources like the Transport Provider Advanced Trauma Course and the Air and Surface Patient Transport: Principles and Practices book.

Jon remains committed to community service, volunteering as a firefighter and training officer, working as a Paramedic for local emergency services, serving as the CFO for a non-profit organization that supports EMS personnel in his region, and serving as an elected official for a tax board in his community.

Jonathan Collier serves as Vice President of Mobile Medicine for Advocate Health, the third-largest nonprofit health system in the United States. In this role, Jonathan leads a diverse portfolio of services that deliver critical care and operational support across multiple states in the Southeast and Midwest. Advocate Health’s Mobile Medicine division encompasses Mobile Integrated Health, communications and dispatch, MED-1 mobile disaster response, transfer center resources, EMS/911, global health initiatives, fleet and logistics, and air and ground transportation through MedCenter Air and AirCare. 

With 25 years of leadership experience in healthcare, Jonathan has held senior roles including Chief Strategy Officer at Apollo MedFlight; Senior Vice President of Operations at Air Methods; Chief Executive Officer at Summit Air Ambulance; and Operations Director and Director of Business Development at PHI Health. Throughout his career, he has focused on building safe, high-quality operations, developing strategy, fostering strong teams, and driving a culture of excellence. Jonathan has partnered with EMS organizations, hospitals, and health systems nationwide to strengthen relationships and improve patient care. He credits his success to exceptional mentors and key partnerships within and beyond healthcare and transport. 

Jonathan serves as President of Aeroterra Health, a transport healthcare utility organization dedicated to delivering essential transport services at the lowest sustainable cost. He is a Commissioner on the Board for Mecklenburg Emergency Medical Services Agency (MEDIC), faculty at the Medical Transport Leadership Institute (MTLI), and a former board member of the Association of Air Medical Services (AAMS). 

He earned a bachelor’s degree in Marketing Communications from Brigham Young University. Jonathan’s greatest professional passion lies in serving patients and supporting underserved communities, alongside his love for aviation. Personally, he enjoys history, languages, and culture, shaped by living in multiple countries. Above all, Jonathan treasures his wife of more than 20 years and their five daughters. 

Jonathon Feit is Co-Founder & CEO of Beyond Lucid Technologies, Inc. He specializes in the development of economic models to financially sustain Community Paramedicine programs. Jonathon contributed to the National POLST Technology Guide on behalf of EMS; and he was the only technologist member of Arizona’s Mobile Integrated Health Network. Jonathon represented Fire & EMS on the Standards Committee of the California Health & Human Services Data Exchange Framework (DXF). His company has deployed America’s first Fire & EMS-interoperable registry of end-of-life medical orders (POLST), and the first statewide registry of pediatric special health needs.  In 2020, he was a UCSF Digital Health Hall of Fame 2020 Nominee; and in 2022, he received a Civilian EMS Award from the California EMS Authority.  He is co-owner of a patent on “post-vehicle crash intelligence.”

Jonathon Larson serves as the EMS Coordinator for the Rockford Fire Department in Rockford, Illinois, where he leads initiatives that bridge emergency medical services, behavioral health, and community-based care. With more than 15 years of experience in emergency services, he has been instrumental in reimagining the department’s Mobile Integrated Health (MIH) and MIH-Mental Health programs—models that emphasize prevention, crisis response, and long-term community wellness.

 Jonathon holds a Master of Public Administration from Northern Illinois University and professional designations as a Certified Emergency Manager (CEM), Illinois Professional Emergency Manager (IPEM), Chief EMS Officer (CEMSO), and Chief Training Officer (CTO). He is also a graduate of the Center for Homeland Security and Defense’s Emergence Program at the Naval Postgraduate School.

 Recognized for his collaborative leadership and data-driven approach, Jonathon regularly presents on topics including EMS innovation, integrated mental health response, and interagency coordination. His work continues to advance the role of public safety in building resilient and connected communities.

Dr. Shapiro is an associate professor and practicing hospitalist at the University of Wisconsin. He is the Medical Director for the UW Home-Based Hospital Care program and has served in this leadership role since the program’s inception. He also serves as a utilization management physician advisor for UW Health, is core faculty of the University of Wisconsin Internal Medicine Residency, and is the physician lead for faculty recruitment and onboarding within the UW Division of Hospital Medicine.

Justin Lemery is the Chief of Emergency Medical Services and Mobile Integrated Healthcare for the Tulsa Fire Department. A second-generation firefighter and an 18-year member, Chief Lemery leads the EMS Branch and the Mobile Integrated Healthcare program one of the largest fire departments in Oklahoma. Chief Lemery is a nationally registered paramedic, a community paramedic and holds a B.S in Fire Administration.  Chief Lemery is involved in various leadership positions that impact community health and well-being such as: International Association of Fire Chiefs EMS Director at Large, International Co-Responder Alliance Vice Chair, Oklahoma Fire Chief Association EMS Committee, Community Health Council, Community Health Improvement Plan advisory leadership,  Region 7 Healthcare Coalition, Emergency Preparedness and Response Team Region 7, and Oklahoma State University-Fire Service Technology adjunct professor. Chief Lemery is currently completing the Executive Fire Officer course at the National Fire Academy and will graduate with a Master’s in Public Administration in Spring of 2026. Chief Lemery is dedicated to advancing emergency medical services and community health through leadership, education, and innovative program development.

Karen has been in EMS for over 30 years with a diverse background in EMS Management, Communications, Education and Quality Assurance and Improvement. She holds degrees in Biology and Homeland Defense and Disaster Preparedness, and Public Health as well as several EMS Instructor certifications.

Karen has extensive experience in EMS Education as a State Certified Instructor Coordinator as well as an Advanced Life Support-level State Examiner.

Ms. Host joined Cataldo Ambulance Service in 2010 as the Quality Assurance and Regulatory Compliance Manager and has since advanced to her current role as Executive Director of Clinical Services/Mobile Integrated Healthcare.

Karen moved into a leadership role with Cataldo Ambulance Service’s launch of Mobile Integrated Healthcare – SmartCare - in 2014 when Cataldo became one of the two authorized pilot programs in Massachusetts. Karen then sat on the state MIH Advisory Council that shaped MIH regulations promulgated in 2019.

Karen has lectured across the country at national MIH conferences and has been recognized by Mass General/Brigham’s as a Pillar of Excellence Award recipient for MIH delivery of monoclonal antibody treatments to home-bound COVID positive patients. Karen is also an elected board member of the Massachusetts Region IV Medical Services Committee

Kristen Hanks is the Director of the Enterprise Ambulatory Care Coordination & Management virtual team located in the Southeast Region of Advocate Health. In this role, she helps lead patients through the care continuum by supporting Primary Care providers, specialty and inpatient partners, while collaborating with key partners like Mobile Integrated Health (MIH). Kristen is a founding nurse of the Enterprise Ambulatory Care Coordination & Management team, instrumental in expanding its reach from Greater Charlotte to the Floyd, Navicent, and Wake markets over the past three years. Her innovative collaboration with MIH has significantly enhanced patient care during transitions from higher levels of care, resulting in improved patient satisfaction, reduced hospitalizations, and decreased emergency department visits.    

Previously, Kristen was a team lead for the COVID virtual hospital and has extensive experience as an emergency room nurse and in outpatient triage, where she identified critical needs for paramedicine in the community. She has also contributed to the development of Cognitive Impairment care pathways for her organization.  

A dedicated professional, Kristen is a member of the American Nurses Association and holds board certification in Ambulatory Care nursing. She is currently pursuing her Master’s degree in Nursing Leadership at Cabarrus College of Health Sciences, where she continues to advance her expertise and commitment to nursing excellence. 

Kristin Davis has been employed with Spartanburg Regional Healthcare System for over 15 years. She began her medical career as a newly minted EMT in the hospital’s ambulance department, where she first realized her passion for helping people. She completed her Paramedic in 2013 and continued her pre-hospital career in the system. She worked as a Supervisor in the Ambulance Services Department for several years but felt like she was missing something while in that role. Thanks to the tremendous success of the Community Paramedic program, the Heart Failure Clinic received a grant to hire a second medic in the summer of 2024. In October 2024, Kristin became Spartanburg Regional Heart Failure Clinic’s second Community Paramedic, primarily serving rural populations. She quickly realized this was her niche and has spent her time there advocating for her patients and assisting them in overcoming barriers they face in their daily lives. With Kristin added to the roster, the number of home patients seen by the Heart Failure Clinic has grown tremendously, and readmissions of these patients have dropped significantly.  

Lara Santos is a registered nurse specializing in wound care and virtual nursing with Advocate Hospital at Home. Her career is rooted in growth, resilience, and a deep commitment to serving underserved and high-risk populations. She began her healthcare journey with Atrium Health in 2009, initially working in patient support and care coordination roles before transitioning into nursing. These early experiences gave her firsthand insight into how access, communication, and trust shape patient outcomes.

In her current role, Lara serves as the in-home clinical eyes and ears for virtual providers, performing wound assessments, supporting treatment plans, and ensuring continuity of care in the home setting. A meaningful part of her work involves educating paramedics and family caregivers, empowering them to confidently deliver safe, effective care beyond traditional clinical environments.

At the heart of Lara’s work is a belief that high-quality care should meet patients where they are—physically, culturally, and emotionally. She is especially passionate about advocating for individuals who are often overlooked by the healthcare system.

Outside of work, Lara is a proud parent to her son, Rowan, who keeps her grounded and continually reminds her why compassionate care matters. She also enjoys traveling to culturally rich places, learning from diverse communities, and gaining perspective that strengthens how she connects with patients and families from all walks of life. These experiences shape her approach to care—one rooted in empathy, respect, and human connection.

Lisa Tripp is an attorney who has more than two decades of experience working with state and federal agencies in the United States on legal matters affecting patients and providers such as hospitals and nursing homes. She co-led the team that created the CMS Acute Hospital Care at Home Waiver in the midst of the COVID-19 public health emergency. The waiver, which allows hospitals to receive the same DRG payment for inpatient care irrespective of whether patients are in their homes or a traditional brick and mortar hospital, has led to a dramatic increase in the adoption of hospital at home across the United States. After leaving government in 2021, Lisa led the advocacy team at Medically Home, which was instrumental in getting Congress to extend the CMS Acute Hospital Care at Home Waiver. In 2024, Shira co-founded Tripp Hollander Advisors, which provides operational, legal and advocacy-related services to advance hospital at home programs and other innovative care delivery models.

Levi is a rare disease advocate impacted by Secondary Intracranial Hypertension, Behcet’s Disease, and Parkinson’s, undergoing 11 major brain surgeries since the initial diagnosis in 2016.

As a former Firefighter/EMT, Levi gained invaluable firsthand knowledge of the healthcare system that would become the building blocks of their patient journey and career. This clinical and practical knowledge was instrumental to diagnosing their own conditions when doctors fell short.

Levi is currently the Storytelling & Community Experience Designer/Lead Patient Senate Officer at Patients Rising but previously served as Patient Navigator for the Patient Helpline assisting other patients with navigating the healthcare system and providing information on federal, state and local resources to equip patients in their access to care needs.

Levi is a Board Member for G-PACT and One Rare, contracts with Medable and participates in podcasts and various learning institutions with the ultimate goal of decreasing the diagnostic odyssey while creating fun and educational content.  Levi is tirelessly working to develop programs and creative solutions to improve the lives and outcomes of fellow patients plus fellow emergency personnel everywhere.

Lori Brown-Loftis is a licensed clinical social worker with a passion for improving access to mental health care, reducing mental health stigma in the community, and creating impactful community programs. Lori has worked in mental health for 17 years including community mental health settings for 13 years. Following that, Lori had the honor of serving our nations veterans for four years at the Oklahoma City VA. Currently, Lori is the program manager for the Oklahoma City Fire Department's Mobile Integrated Healthcare (MIH) Unit. In her current role, Lori has the privilege of leading an innovative team that addresses behavioral health crises, substance use, and social determinants of health through integrated and compassionate care.

Luke Estes holds the position of Manager for the Mobile Integrated Health Program at PRISMA Ambulance, and he is an active member of the Legislative committee within the National Association of Mobile Integrated Health Providers (NAMIHP). Additionally, he serves as the Chairman of the CP/MIH committee for the South Carolina EMS Association. With more than 23 years of Paramedic certification under his belt, Luke has dedicated 6 of those years to working in the field of public and population health.Luke's deep-seated passion lies in disseminating knowledge and enabling individuals to surmount healthcare-related obstacles.

Mandy McGowan is a Registered Nurse in the Madison, WI, area. She is the Director of Home-Based Care Services for UW Health, including Home-Based Primary Care and Home-Based Hospital Care. Mandy has worked in the home health arena for 19+ years and has spent most of that time in various leadership roles, with a focus on integrating home-based care services into the larger health system continuum of care. Prior to home health, Mandy worked as a bedside nurse, charge nurse and flight care nurse in Neonatal ICUs in Madison, WI, and New Orleans, LA.

Margaret Piwowar, PhD, MPhil, MSHE, BSNS, EMT-P, EMT-CP, is a community paramedic in Chicago. With over 25 years of experience in EMS and over 5 years of experience with MIH, she has been instrumental in the evolution of community paramedicine in a major metropolitan area. She is a true believer in health education and earned her PhD in health education with a focus on community outcomes. Outside of work, she volunteers with animals, spends time with loved ones, and enjoys swimming and staying active. She is passionate about increasing health literacy to reduce health disparities for everyone.

Dr. Mary Rudyk is a board-certified internist and geriatrician with more than 30 years of clinical experience. Based in Wilmington, North Carolina, she has built a longstanding private practice dedicated to the comprehensive care of older adults.

She currently serves as Medical Director of Hospital at Home and Medical Director of Home Health, leading innovative models of care that bring hospital-level services and coordinated support directly into patients’ homes. In addition, she provides oversight in several nursing homes, ensuring high-quality, compassionate care for residents.

Throughout her career, Dr. Rudyk has emphasized a continuum-of-care approach, following patients through every phase of their health journey—from outpatient management and hospitalization to post-acute rehabilitation and hospice. She works collaboratively with a dedicated team of nurse practitioners and physician assistants to deliver patient-centered, team-based care.

Matt Lewis oversees EMS operations at Atrium Health, spearheading innovation in 911 and interfacility transport. His dedication to patient-centered care and workforce development drives his efforts to reduce hospital admissions and enhance community health. Through structured change practices, he leads improvements across various healthcare levels, resulting in positive outcomes for both teammates and patients.

Matt is a nationally respected EMS and Mobile Healthcare consultant with PWW Advisory Group (PWW|AG), where he partners with local communities, EMS agencies, fire departments, ambulance services, hospitals, and healthcare organizations to evaluate, design, and strengthen modern EMS and mobile healthcare delivery systems. 

Prior to joining PWW|AG, Matt served as Chief Transformation Officer for MedStar Mobile Healthcare, the nationally recognized Public Utility Model EMS system serving Fort Worth and 13 surrounding cities in North Texas. In this role, he was instrumental in developing and implementing innovative, high-impact programs alongside healthcare and community partners—helping redefine the role of EMS within the broader healthcare ecosystem. 

Matt holds a Master’s degree in Healthcare Administration and a Graduate Certificate in Healthcare Data Management. He is a licensed Emergency Medical Technician (EMT) and a widely recognized leader in the field, having served as Past President of the National Association of Emergency Medical Technicians (NAEMT). He also currently serves as Executive Director of the Academy of International Mobile Healthcare Integration (AIMHI), an association representing high-performing and Public Utility Model EMS systems across the United States and Canada.

Patrick Watson is a 25-year veteran of Fire and EMS services with a distinguished career dedicated to advancing out-of-hospital care. He is the co-owner and consultant at Priority One Innovative Solutions, a firm specializing in healthcare system design, operational strategy, and mobile integrated health innovation.

Patrick is a co-founding Community Paramedic with UVA Health in Charlottesville, Virginia, where he helped design, implement, and operate a nationally recognized Community Paramedicine Program that has reduced emergency department utilization by 72%.

He also serves as the Deputy Chief, Life Member, and Paramedic with the Western Albemarle Rescue Squad in Crozet, Virginia. His extensive background includes experience with UVA Pegasus Air and Ground Services and leadership at the Charlottesville–Albemarle–UVA 9-1-1 Communications Center.

Patrick’s passion lies in bridging healthcare and public safety through innovation, collaboration, and patient-centered system design.

Rick Sauriol is the Network Delivery Manager for instED, liaison for all things relevant to the paramedics' care delivery during acute care visits in the home, such as visit distribution and quality control. Rick had ten years of experience in EMS with the last five also as an MIH paramedic, providing quality, compassionate care in emergency, hospice, hospital-at-home, and in-home acute care ED avoidance programs.

Experience includes front-line supervisory work of MIH paramedics, and many years in operations management for other industries such as landscape/facilities maintenance and residential education.

Rick holds a Bachelor of Arts from Stonehill College, and is a Nationally Registered Paramedic.

Ryan McDonald began working as an EMT in 1992, and attained his paramedic license in 1994. He has spent his EMS career working for services in Central and Southwest Missouri. In 2016, he started his employment with Citizens Memorial Hospital, and has worked as both a field paramedic and as a crew supervisor. He also became heavily involved in EMS education, and has instructed numerous NAEMT courses, as well as nine EMT academies. In preparation for the implementation of a Mobile Integrated Healthcare program, a community paramedic course was offered at CMH at the start of 2023. Ryan completed this training and with that licensure change, onboarded as the second fulltime Community Paramedic with CMH in the beginning of 2024.
The numerous years working EMS in rural Missouri and seeing the growing needs of the community has fueled a sense of compassion and empathy that drives his role as a community paramedic.
 

Ryann Allen serves as the Mobile Integrated Healthcare (MIH) Coordinator at CMH, bringing over seven years of experience in patient care, care coordination, and community health. As a CHW, She is dedicated to advancing Mobile Integrated Healthcare initiatives that improve access, coordination, and outcomes for patients across the community.

 Growing up in a small rural town in Missouri, Ryann developed a strong sense of community, empathy, and compassion that inspired a lifelong commitment to helping others. These values continue to guide her work today. Outside of work, she enjoys spending time with her husband and their almost 3 year-old son, exploring the outdoors, spending time with family, and creating lasting family memories.

As the MIH Coordinator, Ryann is passionate about innovation, connecting high risk patients and families with the resources they need, reducing avoidable ER visits, improving access to care in rural communities, and ensuring that patients in need receive needed care and support.

Sabrina Ballew is the Manager of Mercy Flights’ Mobile Integrated Healthcare (MIH) Program and Chair of the Oregon MIH Coalition. A Certified Community Paramedic (CP-C) and Community Health Worker (CHW), she has been instrumental in shaping one of Oregon’s most comprehensive and community-driven MIH programs. Sabrina’s leadership blends innovation, compassion, and data-driven strategy to advance care coordination, reduce preventable emergency utilization, and improve health equity across Southern Oregon.

With over a decade of experience in community-based EMS and healthcare system transformation, Sabrina has guided state and national conversations around sustainable MIH funding, workforce development, and CAMTS accreditation readiness. She also serves on the boards of the National Association of Mobile Integrated Healthcare Providers (NAMIHP) and the Commission on Accreditation of Medical Transport Systems (CAMTS) MIH Advisory Board. Her passion lies in building resilient, patient-centered systems that connect emergency medicine, primary care, and behavioral health—meeting people where they are and redefining what accessible care looks like in every community.

Samantha Martinez, MSN, RN, EMT-P, is a dedicated healthcare professional with extensive experience in emergency and pre-hospital medicine. She began her healthcare career as a Certified Nursing Assistant (CNA) in 2011 and became a licensed Paramedic in 2015, gaining valuable field experience working on private ambulances. In 2017, she earned her Master of Science in Nursing (MSN), further advancing her clinical expertise and expanding her role in patient care. Since 2020, Samantha has proudly served with the fire department, where she continues to integrate her nursing knowledge and emergency response skills to meet the evolving needs of her community.

Her professional experience bridges the gap between acute and preventive care, with a passion for connecting patients to the education and resources they need to manage chronic conditions effectively. Drawing on her emergency medicine roots, she brings a calm, compassionate, and solution-oriented approach to every clinical interaction.

Sarah Lewis has been a dedicated member of Fairfield Medical Associates (FMA), a rural health clinic serving three counties in South Carolina. She has served in numerous roles within the organization that include clinical, billing and administration/leadership since joining the team in 2008 and she currently serves as the Clinical Manager. With a deep passion for population health initiatives, she has been instrumental in leading the organization's efforts to enhance community health outcomes. Sarah is the innovator and lead for the community paramedic pilot program which is a collaborative initiative with the South Carolina Emergency Medical Services Association (SCEMSA). This program aims to provide proactive, patient-centered care in the community, focusing on individuals with chronic conditions, frequent hospital readmissions, or those recently discharged from medical facilities by embedding a community paramedic within FMA. Sarah is committed to improving healthcare delivery in rural communities through partnerships and is a strong advocate and leader of rural healthcare. Her work exemplifies the impact of innovative healthcare models in addressing the unique challenges faced by underserved populations.

Scott Goldberg, MD, MPH, FACEP, FAEMS, is faculty in the Department of Emergency Medicine at Brigham and Women’s Hospital in Boston, MA where he serves as the Director of EMS for Mass General Brigham and as the medical director for emergency preparedness at Brigham and Women’s. He holds ABEM board certification in both emergency medicine and emergency medical services. Clinical work includes mass gathering medicine and special operations EMS, providing medical support and oversight for Gillette Stadium as well as several other regional and national large mass gathering events. Research interests include empowering community response to life threatening emergencies, including bleeding control, opioid overdose, and out of hospital cardiac arrest. He currently serves as the chair of the Mobile Integrated Health Committee with the National Association of EMS Physicians.

Scott Smith has been employed by the Spartanburg Reginal Healthcare System (SRHS) for 15 years. He began his career in healthcare as a new EMT at a local private ambulance service, and transitioned to the Ambulance Services department in a part time roll 6 months later. He transitioned into a full-time roll at Ambulance Services approximately 6 months later. After a couple of years in an EMT role he desired to have more of an impact in Health care and received a Paramedic certification in 2013 and he continued his career in the hospital setting . He served in a Crew Chief role in Ambulance Services, and as a Senior Paramedic for 10 years working third shift. While working in the hospital setting, he started to realize there was something missing and desired to have more of an impact in his role as a paramedic. As the success of the Community Paramedic program grew at SRHS, he realized this was the setting where he desired to contribute. In July of 2025 a decision was made to hire a third community paramedic for the Community Paramedic Program at SRHS Heart Failure Clinic. In August of 2025 he was hired and began advocating for patients and assisting them in overcoming heart failure complications and thriving in their daily lives. 

Shannon Shanks is a nurse practitioner and supervises the medical team at Manatee County Emergency Services Community Health Program.  She earned her Doctoral degree (DNP) from Lourdes University in Sylvania Ohio. Her Master of Science in Nursing (MSN) with a concentration in Family Nurse Practitioner were obtained from Herzing in Madison Wisconsin.   Her undergraduate degrees were obtained from Mercy College of Northwest Ohio, in Toledo Ohio.

Dr. Shanks has over twenty-seven years of healthcare experience. Ranging from prehospital emergency medicine as a paramedic, Level I, II trauma community level emergency medicine, primary care, and education. Most recently has been working in mobile integrated health care.

For the last two and a half years, Dr Shanks has led the medical team at Community Health, a mobile integrated health program in Manatee County.  She and her team work together in street medicine, addiction medicine and address many the social determinates and health needs of the underserved population.

She and her collaborative team are recipients of the 2023 18th Congressional District Fire Rescue/EMS for work in the community, as well as the 2024 Florida Department of Health Mobile Integrated Healthcare program of the year.   They were also the second MIH program to become fully accredited through CAMT

After a career in academia Shawn Murphy began working in EMS in 2011 in both the pre-hospital and emergency room settings and is a trained critical care paramedic. In 2020 he became a member of the Chicago Fire Department and subsequently trained as a community paramedic and joined the department’s Mobile Integrated Health Unit (MIHU) where he currently serves. In addition to his full-time work as a community paramedic, Shawn continues to teach as adjunct faculty at DePaul University and as an instructor in the medical school at the University of Illinois, Chicago.

Shelby VanDerMoere is the Mobile Integrated Health Program Manager and Community EMT for the City of Bloomington Fire Department. Shelby began her journey with BFD in October 2022 where she provides gap care for patients before and after 911 calls. With a Bachelor of Science in Human Services from Purdue University Global, a certified Community Health Worker, and as a nationally registered EMT, Shelby is uniquely positioned to provide holistic care to the community in their place of need.

Shelby was the recipient of the 2023 Indiana EMS Specialty Care Provider of The Year award presented by the Indiana Fire Chiefs Association, and her team, also in 2023, was awarded the “Most Progressive Public Safety Innovation” by the City of Bloomington. She received the 2024 Outstanding Caregiver by The Community Dementia Alliance of Bloomington, and in 2024 and 2025 Shelby was named a Mental Health Champion for Monroe County Nominee.

Today, Shelby serves on the Commission on the Status of Children and Youth for the City of Bloomington where she has been able to carry on her passion for creating and supporting healthy families and is on the Board of Directors for the National Association of Mobile Integrated Health Providers

Shira Hollander, JD/MPP, is a health care advocate, attorney and strategist with over a decade of experience developing policy, advocating for patients and providers, and advising hospitals, health systems, payors, and pharmaceutical companies on compliance with health care rules and regulations. After several years in private legal practice, Shira joined the American Hospital Association in 2017, where she led AHA’s advocacy, strategy, and policy work on telehealth, value-based payment and innovations in care delivery. In 2022, Shira joined the advocacy team at Medically Home, which was instrumental in getting Congress to extend the CMS Acute Hospital Care at Home Waiver. In 2024, Shira co-founded Tripp Hollander Advisors, which provides operational, legal and advocacy-related services to advance hospital at home programs and other innovative care delivery models.

Steve Keller has over 35 years of EMS experience. He began his career in 1990 at Citizens Memorial Hospital as a staff EMT. He obtained his Missouri paramedic license in 1992. Steve spent ten years as a flight paramedic for CoxHealth System in Southwest Missouri. He also served as director of EMS and Assistant Director of Emergency Management for CMH for eleven years. Steve moved from his director position to a field supervisor position and also supervised QA/QI for EMS. He completed his community paramedic training through Mineral Area Community College in the latter part of 2022. Steve became the first CMH Mobile Integrated Healthcare community paramedic in August of 2023.
 

Taylor Tereskerz is a dedicated EMS professional and healthcare innovator with over a decade of experience in emergency medical services. He is the co-owner and consultant at Priority One Innovative Solutions, a firm specializing in healthcare system design, operational strategy, and mobile integrated health innovation.

Taylor began his EMS career in 2011 with the Western Albemarle Rescue Squad, where he now serves as a Captain, EMT-Intermediate, and Life Member within Albemarle County Fire Rescue system. He holds a Bachelor’s degree in Health Sciences Management from the University of Virginia and maintains certifications as an EMT-Intermediate, among others.

In 2019, Taylor joined UVA Health’s Population Health Department, working to reduce hospital readmissions through patient-centered care coordination. In 2022, he became one of the co-founders of the UVA Health Community Paramedicine Program, developing the initiative from the ground up. The program has since achieved a 72% reduction in emergency department utilization, demonstrating its significant impact on patient outcomes and healthcare efficiency.

Taylor is passionate about advancing the future of Mobile Integrated Healthcare and is eager to share his experiences, insights, and lessons learned to help shape innovative, patient-focused care models.

Trisha Rademachir serves as a Community EMT with the City of Bloomington’s Mobile Integrated Health (MIH) Program, where she provides gap care to underserved populations and helps bridge barriers between patients and the healthcare system. Her compassion for community health is matched by her skill as the team’s grant writer, working diligently to equip the MIH team with resources that enhance care and create lasting impact.

Trisha’s journey into MIH began while preparing to lecture on the topic, where she discovered a deep calling to serve her community beyond the classroom. A chance conversation led her to the program, a role she now calls a privilege, working alongside some of the most dedicated professionals in the field.

Trisha holds Associate Degrees in Homeland Security and General Studies and is a Nationally Registered EMT, Primary Instructor, and certified Community Health Worker. She is Perinatal Mental Health Certified and has completed advanced training in Chronic Care, Opioid-Informed Family Support, and Domestic Violence.

In 2024, Trisha was honored as the Indiana EMS Specialty Care Provider of the Year by the Indiana Fire Chiefs Association.

Tyler Jackson is an experienced healthcare operations leader with a strong background in Mobile Integrated Healthcare (MIH) and telemedicine services. As Operations Manager for North State Medical Transport’s MIH division, Tyler oversees advanced in-home patient care, including complex treatment plans, IV infusions, wound care, and diagnostic testing for UNC’s Advanced Care at Home program. Previously, he served as Operations Manager at LoginClinics, where he implemented telemedicine solutions, ensured regulatory compliance, and managed employee training and safety protocols. With over a decade of experience as a paramedic and healthcare professional, Tyler brings a proven track record of improving patient outcomes and operational efficiency.
