    Alberto Uscanga is currently a Heavy Equipment Operator-Paramedic and Mobile Integrated Health Care Practitioner with the Milwaukee Fire Department. In his current role, he serves the greater Milwaukee area in a recently expanded role in Milwaukee’s Overdose Response Initiative (M.O.R.I.). Tasked with continuing to support the area’s most vulnerable populations by actively engaging those experiencing substance use, his team is also tasked with forging meaningful connections with businesses, community organizations and other entities in some Milwaukee’s most active areas.

    HEO Uscanga joined the ranks of the Milwaukee Fire Department as a Firefighter in November of 2019. He brought with him experiences in the private EMS sector as well as smaller community fire departments near and far from his native Milwaukee. Having earned his Paramedic Certification from Lakeshore Technical College in 2018, his foundation in Jesuit education from both Marquette University and Marquette University H.S. laid the groundwork for him today as a ‘man for others’.

    Wasting little time, then Firefighter Uscanga became immersed in all that Milwaukee Fire could provide. Joining Tactical EMS, Incident Command Post, Hazmat, Recruitment, and serving as Fire Education Specialist prior to promoting to HEO. In the midst of this rapid growth, HEO Uscanga seized the opportunity to join the MIH team as a technician, working on his days away from the firehouse. He rapidly found a place to apply his high energy and deep desire to be the difference in peoples lives.

    After obtaining his Community Paramedic certification from UWM College of Nursing in 2020 and honing those skills as an MIH lead, he was asked to join the MIH/MORI team full time in June of 2024.

    Since then, HEO Uscanga continues the mission he and those at Milwaukee Fire Department have committed to; “to utilize all the necessary resources, at our command, to provide a quality of service deemed excellent by our citizens, with ‘courage, integrity, and honor…’”.

    With over 20 years of nursing experience, Amanda Henley has dedicated her career to providing exceptional patient care across various clinical settings. She earned her Associate Degree in Nursing from Gaston College and her Bachelor’s Degree from Appalachian State University.

    Amanda began her nursing journey in 2004 as a bedside nurse in a Level 4 Neonatal Intensive Care Unit in Charlotte, NC, where she developed a strong foundation in caring for critically ill infants. Her passion for trauma nursing led her to the emergency department of a Level 1 trauma center, where she honed her skills in high-pressure environments.

    In 2008, Amanda joined MedCenter Air as a critical care flight nurse, delivering advanced medical care to patients ranging from neonates to the elderly during transport via ground, helicopter, and fixed-wing aircraft. While serving as a flight nurse, she further expanded her expertise by obtaining her National Registry Paramedic certification.

    After a decade in transport nursing, Amanda transitioned to a trauma program coordinator role, where she standardized trauma care practices at a Level 3 trauma center. This pivotal experience paved the way for her position as an administrative house supervisor, overseeing patient care operations at Levine Children's Hospital, a leading Level 1 pediatric trauma center in Charlotte.  This led to Amanda becoming a nurse manager where she successfully led nursing teams in the inpatient acute care hospital setting.

    Currently, Amanda serves as a Nurse Navigator on Atrium Health's Hospital at Home Team. In this capacity, she is instrumental in the initial admission of patients requiring ongoing hospitalization, facilitating their transition to home-based care with a mobile hospital team. Her extensive background equips her with a holistic perspective on patient care, enabling her to accurately assess complex medical and psychosocial needs while providing essential education for safe and successful transitions to hospital-level home care.

    Andrew Ross has been working in EMS for over 23 years. He obtained his EMT certification from Central Piedmont Community College and later attended Gaston College for his paramedic. After spending a few years on an ALS unit, he was promoted to the role of preceptor, where he discovered his love of teaching. Andrew has since served as a field training officer, shift supervisor, training director, assistant director, and as an operations supervisor for a large multi-state EMS service. He now is a full-time faculty member at Central Piedmont Community College, serving as the EMS Program Director of Emergency Medical Sciences. His research interests include healthcare management, application of educational theory, bioethics, and community paramedicine.     Dr. Frank is an actively practicing, board certified emergency physician with decades of experience in emergency medicine and emergency medical services. He is sub-specialty board certified in Undersea and Hyperbaric medicine and wound care. He has maintained an active North Carolina EMT-Paramedic certification since 1997. Dr. Frank currently serves as the Chief Medical Officer and Senior Vice President for Medical Affairs for UNC Health Blue Ridge and as the Burke County EMS Medical Director in Morganton, NC. Dr. Frank is a graduate of Washington and Lee University, Eastern Virginia Medical School and completed residency training in Emergency Medicine at the University of South Carolina. Dr. Frank completed his Masters of Business Administration from the University of Massachusetts at Amherst in 2018, and is a Certified Physician Executive by the American Association for Physician Leadership.

    Dr. Frank began his medical career as a volunteer EMT in high school in Virginia in 1984 progressing to a EMT – Cardiac during college and volunteered in EMS through medical school. He has served in multiple EMS Medical Director roles throughout residency and in active medical practice currently serving as the EMS / Community Paramedicine (CP) Medical Director for Burke County EMS and as the medical oversight for a hospital based mobile integrated health (MIH) program at UNC Health Blue Ridge. As a physician over the last 30 years, he has had significant opportunities to develop relevant experience in healthcare and EMS leadership, academic emergency medicine, hospital operations, and graduate and paramedic medical education programs. Dr. Frank has extensive leadership experience in physician and EMS Development, leadership, ED and EMS operations, management, and process improvement in complex healthcare organizations and across hospital systems in facilities and emergency departments ranging from 10,000 – 120,000 ED visits a year. He has experience in urban and rural EMS systems.

    Christopher Goenner, originally from Pennsylvania, brings over 34 years of experience in public safety to his role as a speaker. He began his career as a volunteer firefighter, EMT, and law enforcement officer before dedicating a decade to full-time EMS education. After relocating to North Carolina in 2007, he earned both his North Carolina and National Registry Paramedic certifications. Chris holds an Associate in Applied Sciences in EMS from Lenoir Community College, a Bachelor of Science in Emergency and Disaster Management, and a Master's in Health Sciences in EMS Education from Western Carolina University.

    Chris has served as a field training officer at Cabarrus County EMS and recently held the position of Department Chair of Emergency Medical Sciences at Central Piedmont Community College. He is an active presenter at local, state, and national EMS conferences, has published research, and received a national award for his work on End-Tidal Carbon Dioxide in septic patients. A former president of the North Carolina EMS Educators Association, he participates in several committees with the North Carolina Office of EMS and has taught as adjunct faculty at Rowan-Cabarrus Community College, Stanly Community College, and the University of North Carolina at Charlotte. He is currently the Program Chair and Associate Professor for the Bachelor of Science program at Cabarrus College of Health Sciences.

She leads the team's abilities to deliver comprehensive patient care, improve patient access, and enhance preventive health initiatives. The team’s partnership with Mobile Integrated Health (MIH) has led to reduced healthcare costs, improved health outcomes, and increased patient satisfaction by having options to deliver care in the home. Since joining the Primary Care Connect team in 2021, Courtney has expanded the team’s support for outpatient clinics across four markets. In 2023, she assumed leadership of the Annual Wellness Visit Nurse team and is actively working to extend this program throughout the Southeast.

    With over a decade of experience as a nursing leader within Atrium Health's medical group, Courtney possesses extensive knowledge in family medicine, internal medicine, and pediatrics. Her previous roles include serving as a regional patient safety coordinator and as a clinical supervisor for the high-risk postpartum unit at Atrium Health Carolinas Medical Center.

    Courtney is currently pursuing her Doctor of Nursing Practice (DNP) in Executive Healthcare Leadership at the University of South Carolina. She holds a master’s degree in nursing leadership from Cabarrus College of Health Sciences and is board certified in Ambulatory Care.

    Cindy Irving started her career in Law Enforcement at the Marion County Sheriff’s Office as a Law Dispatcher/TTY Operator/911Telecommunicator in January of 2000. In 2005, Cindy became a victim advocate, and in 2008 became a Crisis Intervention Specialist. On April 20, 2018, appointed by Sheriff Billy Woods, Cindy helped develop the Juvenile Intervention Specialist position after the Forest High School shooting. In October of 2023, Cindy Irving became the Senior Intervention Specialist with the Marion County Sheriff’s Office and a part of the Mobile Integrated Health Team.

    Cindy is a Victim Practitioner through the Office of the Attorney General and has been working with victims of crime for the past nineteen years. She has been teaching the Personal Awareness/Self Defense classes since becoming Victim Advocate in 2005. In 2018, Cindy became a certified Rape Aggressive Defense (RAD) Instructor. She has recently become a Peer Support Team member to assist coworkers in different circumstances.

    Cindy has an AA in Psychology from Central Florida Community College, a BA in Psychology from the University of Central Florida, and a Masters in Social Work from Simmons College. Cindy has served as co-chair on the Marion County Children’s Alliance Family Violence Prevention Workgroup. Cindy currently serves on the Baker Act Task Force, Continuum of Care, Public Policy Institute, Local Review Teams for juveniles who need services, and other committees.     Jonathan Belott is currently a Fire Lieutenant-Paramedic and Mobile Integrated Healthcare Program Manager with the Milwaukee Fire Department (MFD).  He is responsible for guiding Mobile Integrated Healthcare (MIH) program operations and team members so they are best able to accomplish their mission of increasing public health within Milwaukee while reducing per-capita costs.   

    Lieutenant Belott was appointed to the MFD as a firefighter in October 2011 before promoting to Heavy Equipment Operator and obtaining his Paramedic certification in September 2014. Lieutenant Belott joined the second class of Community Paramedic graduates from the training program provided by the School of Nursing at UW-Milwaukee in 2017.  

    As the Community Paramedic program expanded, the MFD reopened a decommissioned firehouse to staff and house the expanding team.  In March 2019 Lieutenant Belott transferred to this newly repurposed firehouse to work full-time as a MIH Practitioner.  Within months of arriving there, Lt Belott was tasked with developing the new Milwaukee Overdose Response Initiative (MORI) and became the first MORI Supervisor.  He was promoted to Lieutenant in February 2020 and became the MIH-Supervisor later that year. Following the retirement of his predecessor, Lieutenant Belott assumed the role of MIH Program Manager. 

    Lt Belott obtained a BFA in Music Education with a business minor from UW-Milwaukee in 2005.  He still actively plays tuba with various musical groups in the area.  He also enjoys taking his camper out with his family to Wisconsin state parks as often as he can throughout the nice weather months.

    Karen moved into a leadership role with Cataldo Ambulance Service’s launch of Mobile Integrated Healthcare – SmartCare - in 2014 when Cataldo became one of the two authorized pilot programs in Massachusetts. Karen then sat on the state MIH Advisory Council that shaped MIH regulations promulgated in 2019.

    Karen has lectured across the country at national MIH conferences and has been recognized by Mass General/Brigham’s as a Pillar of Excellence Award recipient for MIH delivery of monoclonal antibody treatments to home-bound COVID positive patients.     Previously, Kristen was a team lead for the COVID virtual hospital and has extensive experience as an emergency room nurse and in outpatient triage, where she identified critical needs for paramedicine in the community. Kristle is solely based in the Heart Failure Clinic with the goal of seeing heart failure patients discharged from the hospital within 7 days. She has been seeing patients since October 2022 and has assisted in a drastic 70% reduction of emergency visits and admissions for heart failure patients enrolled in the program. Recently passing the 2-year anniversary of the program's inception, Kristle has enrolled 150 patients and performed 800 home visits.

    Shelby VanDerMoere is the Mobile Integrated Health Program Manager and Community EMT for the City of Bloomington Fire Department. Shelby began her journey with BFD in October 2022 where she provides gap care for patients before and after 911 calls. With a Bachelor of Science in Human Services from Purdue University Global, an Associate of General Studies in General Studies, a certification for a Community Health Worker, and as a nationally registered EMT, Shelby is uniquely positioned to provide holistic care to the community in their place of need. 

     

    Previously, Shelby served as the victim’s advocate for a prosecutor’s office for five years where she honed her skills in advocacy, ultimately preparing her for the patient advocate work she does today.  She serves on the Commission on the Status of Children and Youth for the City of Bloomington where she has been able to carry on her passion for creating and supporting healthy families.

     

    This past year, Shelby was the recipient of the 2023 Indiana EMS Speciality Care Provider of The Year award presented by the Indiana Fire Chiefs Association, and her team was given an Innovation Award for “Most Progressive Public Safety Innovation” by the City of Bloomington. Most recently in 2024, Shelby was named a Mental Health Champion for Monroe County Nominee and the 2024 Outstanding Caregiver by The Community Dementia Alliance of Bloomington and IU Health Foundation, ADRS. 

     

    When Shelby takes off her professional hat, you will likely find her wearing some sort of sports mom gear cheering on her kiddos. She has a beautiful blended family with 4 children and a husband that serves right alongside her. Shelby enjoys baking, podcasts, and jeep rides.

    Taylor is a Crozet, Virginia native through and through. Taylor has been in EMS since 2011 in various capacities, starting at the Western Albemarle Rescue Squad. Taylor is a Medic, Captain, and lifetime member of the esteemed Western Albemarle Rescue Squad within Albemarle County Fire Rescue. His array of certifications, include, EMT-I, Swift Water Rescue, Emergency Vehicle Operator Class III, and Vehicle Extrication, Advanced Cardiovascular Life Support (ACLS) and Pediatric Advanced Life Support (PALS). Taylor currently holds an Associates in general studies from Piedmont Virginia Community College, and is slated to graduate with a bachelor’s degree in Health Sciences Management from the University of Virginia in the spring of 2025. Taylor previously worked as a full-time medic with the UVA Special Event Medical Management (SEMM) team starting in 2014. In 2019 Taylor transitioned into the UVA Population Health department in a remote role working with patients to decrease readmissions. In 2022 Taylor was one of two people in the Population Health Department that was moved into the Community Paramedicine role. Together Taylor and his partner developed the UVA Health, Community Paramedicine program from the ground up. Taylor has been seeing patients in this role for the last 2 years which touts the extremely encouraging data of a >70% decrease in ER visits. Taylor is beyond excited about this new opportunity in EMS and what it can mean for providers and patients alike. He is thrilled to share advice, experiences, and knowledge in this new field as the work is invaluable

    Thomas Striegel, DO, FACEP is an Assistant Clinical Professor of Emergency Medicine at University of Iowa Hospitals and Clinics, Iowa City, Iowa where he is faculty for the Emergency Medicine Residency and the Carver College of Medicine. He was previously faculty at the University of Arizona College of Medicine in Phoenix where he served as Emergency Medicine Clinical Clerkship Co-Director. He is board certified in Emergency Medicine and Hospice/Palliative Medicine and aligns the two specialties with teaching and research in open-hearted communication, patient/physician bias affecting medical decision making and healing caregiver emotional trauma. Dr. Striegel also collaborates within the EMS Division, developing Mobile Integrated Health – Paramedicine (MIH-P) programs locally with Johnson County Ambulance Service as their MIH-P Medical Director and statewide as Iowa’s MIH-P Development Co-Chair. He works with Ceribell Point of Care EEG conducting innovative emergency medicine research.

    Tiffany Vaughn, RN, BSN, has been an RN for almost 15 years, primarily in cardiac nursing. Following 7 years of bedside nursing she spent 5 years at a private cardiology practice working for a cardiologist specializing in Structural Heart Procedures. During 12 years in both inpatient and outpatient cardiac care, Tiffany saw first-hand how a patient's social determinants of health impacted their outcomes. Tiffany's more recent role as the HFC Nurse Navigator has given her the opportunity to do what she loves-advocate. She strives to advocate for her patients to get the best available care possible. As Nurse Navigator her primary goal is to "find" patients that are hospitalized with Heart Failure and provide 7 day follow up at the HFC. While seeing these patients she tries to get to the root cause of why they have multiple HF hospitalizations. For patients with significant social determinants of health, she recommends the HFC MIH-CP program. She then sets up Telehealth visits coordinating the Paramedic and the Nurse Practitioner at whichever HFC the patient lives closest to.

    Todd Barton has served as Mayor of Crawfordsville since January 1, 2012 and is currently serving his fourth term.

    Mayor Barton is a native of Crawfordsville and Montgomery County and served the community for 26 years as a fire fighter/paramedic with the Crawfordsville Fire Department.

    He graduated with honors from Wabash College in 2000 with a B.A., majoring in political science. He earned his degree in three years by attending the liberal arts school full-time, working full-time and raising a family.

    As mayor he has led an ambitious and successful effort to restore financial stability, rebuild infrastructure, improve quality of life and modernize economic development efforts through business retention, attraction and workforce development.

    Under his leadership several innovative programs have been developed in Crawfordsville that have received statewide, national and international attention. These include Mobile Integrated Health, Project Swaddle, Project Bundle, Quick Response Team to address addiction and mental health, Creation of a Workforce Alliance, the Montgomery County Early Childhood Coalition, a monthly community forum, and a weekly podcast.

We have two teams, each made up of a firefighter paramedic and nurse practitioner. Our program is a three-way partnership with our local hospital, our federally qualified health center, and our fire department. We provide primary care services to Olathe community members at no cost to them, wherever they need it. These services also include referrals to behavioral health services and substance use treatment.
