Alberto Uscanga is currently a Heavy Equipment Operator-Paramedic and Mobile Integrated Health Care Practitioner with the Milwaukee Fire Department. In his current role, he serves the greater Milwaukee area in a recently expanded role in Milwaukee’s Overdose Response Initiative (M.O.R.I.). Tasked with continuing to support the area’s most vulnerable populations by actively engaging those experiencing substance use, his team is also tasked with forging meaningful connections with businesses, community organizations and other entities in some Milwaukee’s most active areas.

HEO Uscanga joined the ranks of the Milwaukee Fire Department as a Firefighter in November of 2019. He brought with him experiences in the private EMS sector as well as smaller community fire departments near and far from his native Milwaukee. Having earned his Paramedic Certification from Lakeshore Technical College in 2018, his foundation in Jesuit education from both Marquette University and Marquette University H.S. laid the groundwork for him today as a ‘man for others’.

Wasting little time, then Firefighter Uscanga became immersed in all that Milwaukee Fire could provide. Joining Tactical EMS, Incident Command Post, Hazmat, Recruitment, and serving as Fire Education Specialist prior to promoting to HEO. In the midst of this rapid growth, HEO Uscanga seized the opportunity to join the MIH team as a technician, working on his days away from the firehouse. He rapidly found a place to apply his high energy and deep desire to be the difference in peoples lives.

After obtaining his Community Paramedic certification from UWM College of Nursing in 2020 and honing those skills as an MIH lead, he was asked to join the MIH/MORI team full time in June of 2024.

Since then, HEO Uscanga continues the mission he and those at Milwaukee Fire Department have committed to; “to utilize all the necessary resources, at our command, to provide a quality of service deemed excellent by our citizens, with ‘courage, integrity, and honor…’”.

Worked full time as an EMT-Basic for 5 years both on an ambulance and as an Emergency Department Tech. Graduated from Pacific University with a Masters of Physician Assistant Studies in 2011.  Worked full time in the Emergency Department as a Physician Assistant for 7 years full-time in both community medical centers and major trauma hospitals.  Worked full time for 5 years with a mobile urgent care service providing care at a patient’s home, facility, or other location.  Currently, work full time with West Metro Fire Rescue for over 2 years in the Mobile Integrated Health program providing treat-in-place services and community outreach.   

With over 20 years of nursing experience, Amanda Henley has dedicated her career to providing exceptional patient care across various clinical settings. She earned her Associate Degree in Nursing from Gaston College and her Bachelor’s Degree from Appalachian State University.

Amanda began her nursing journey in 2004 as a bedside nurse in a Level 4 Neonatal Intensive Care Unit in Charlotte, NC, where she developed a strong foundation in caring for critically ill infants. Her passion for trauma nursing led her to the emergency department of a Level 1 trauma center, where she honed her skills in high-pressure environments.

In 2008, Amanda joined MedCenter Air as a critical care flight nurse, delivering advanced medical care to patients ranging from neonates to the elderly during transport via ground, helicopter, and fixed-wing aircraft. While serving as a flight nurse, she further expanded her expertise by obtaining her National Registry Paramedic certification.

After a decade in transport nursing, Amanda transitioned to a trauma program coordinator role, where she standardized trauma care practices at a Level 3 trauma center. This pivotal experience paved the way for her position as an administrative house supervisor, overseeing patient care operations at Levine Children's Hospital, a leading Level 1 pediatric trauma center in Charlotte.  This led to Amanda becoming a nurse manager where she successfully led nursing teams in the inpatient acute care hospital setting.

Currently, Amanda serves as a Nurse Navigator on Atrium Health's Hospital at Home Team. In this capacity, she is instrumental in the initial admission of patients requiring ongoing hospitalization, facilitating their transition to home-based care with a mobile hospital team. Her extensive background equips her with a holistic perspective on patient care, enabling her to accurately assess complex medical and psychosocial needs while providing essential education for safe and successful transitions to hospital-level home care.

Amber Obert has served as a FF/Medic with the James City County Fire Department for over 10 years. She is an Advanced Care Paramedic and moved over to the CONECT program last year. Her passion is advocating for and educating our citizens to become self-sufficient with their healthcare needs. Amber earned her Associates in Occupational Science from Centura College in 2011.She enjoys spending time with her husband and four beautiful children, visiting new places, focusing on nutrition and working out.

Amy Schiotz is the Director of Client Growth for Best Buy Health. Amy has over two decades of experience in patient monitoring, diagnostics, and digital technology and held various leadership roles at Masimo, Mayo Clinic, and Microsoft. In her current role, Amy supports healthcare providers define their strategy around the patient care continuum from wellness to aging to care in the home using technology and enhanced services. She practiced as a respiratory therapist early in her career working Level I trauma and ICU and completed her MBA in Healthcare Administration from University of Southern Indiana.

Andrew Ross has been working in EMS for over 23 years. He obtained his EMT certification from Central Piedmont Community College and later attended Gaston College for his paramedic. After spending a few years on an ALS unit, he was promoted to the role of preceptor, where he discovered his love of teaching. Andrew has since served as a field training officer, shift supervisor, training director, assistant director, and as an operations supervisor for a large multi-state EMS service. He now is a full-time faculty member at Central Piedmont Community College, serving as the EMS Program Director of Emergency Medical Sciences. His research interests include healthcare management, application of educational theory, bioethics, and community paramedicine. When he isn't teaching he is spending time with his family and playing music.

Dr. Frank is an actively practicing, board certified emergency physician with decades of experience in emergency medicine and emergency medical services. He is sub-specialty board certified in Undersea and Hyperbaric medicine and wound care. He has maintained an active North Carolina EMT-Paramedic certification since 1997. Dr. Frank currently serves as the Chief Medical Officer and Senior Vice President for Medical Affairs for UNC Health Blue Ridge and as the Burke County EMS Medical Director in Morganton, NC. Dr. Frank is a graduate of Washington and Lee University, Eastern Virginia Medical School and completed residency training in Emergency Medicine at the University of South Carolina. Dr. Frank completed his Masters of Business Administration from the University of Massachusetts at Amherst in 2018, and is a Certified Physician Executive by the American Association for Physician Leadership.

Dr. Frank began his medical career as a volunteer EMT in high school in Virginia in 1984 progressing to a EMT – Cardiac during college and volunteered in EMS through medical school. He has served in multiple EMS Medical Director roles throughout residency and in active medical practice currently serving as the EMS / Community Paramedicine (CP) Medical Director for Burke County EMS and as the medical oversight for a hospital based mobile integrated health (MIH) program at UNC Health Blue Ridge. As a physician over the last 30 years, he has had significant opportunities to develop relevant experience in healthcare and EMS leadership, academic emergency medicine, hospital operations, and graduate and paramedic medical education programs. Dr. Frank has extensive leadership experience in physician and EMS Development, leadership, ED and EMS operations, management, and process improvement in complex healthcare organizations and across hospital systems in facilities and emergency departments ranging from 10,000 – 120,000 ED visits a year. He has experience in urban and rural EMS systems.

In this role as Deputy Chief in charge of the Mobile Integrated Healthcare and Fire Prevention Bureaus
for the City of West Allis Fire Department, he is responsible for overseeing various aspects of the 
department's operations, with a particular focus on community risk reduction. He has been instrumental 
in developing and implementing innovative programs that bridge the gap between healthcare providers 
and the community, ensuring that residents receive the highest quality care.
Deputy Chief Suarez Del Real has made significant contributions to the West Allis Fire Department and 
the community he serves throughout his career and has demonstrated a deep understanding of the 
complexities of pre-hospital care, critical incident management, and community paramedicine.

I am the Chief Partner-Client Officer for Beyond Lucid Technologies. I have been a Paramedic for over 32 years and worked in EMS for over 33.  During my career I have spent time in both the role of provider and management in both small rural systems to time with New York City EMS and Magen David Adom in Israel.

I have been involved in many programs in the area of MCI preparedness and response; serving as the Planning Section Chief for CT region 3 Incident Management Team deployed with the team in that role to many incidents including the Clean Energy Plant Explosion and Sandy Hook Shooting to name a few.  I also served as a member of the Integrated EMS Response to Active Shooter work group as part of the Connecticut Capital Region. I was awarded the CT Public Health Commissioner’s Award for Individual Impact on EMS in the State of CT.  I have been the author of many EMS related articles as well.

My name is Ashley Pierce and I am an  associate degree Paramedic with 11 years of experience. I have an extreme passion for pediatric patients. My love for pediatrics started early in life due to my family history with special needs children. I work as Community Paramedic with Atrium Health MIH in Charlotte, NC. My job is a big part of my life, and I love meeting new people and exploring new opportunities, outside of work I enjoy spending time with my husband and our two beautiful girls hiking and camping around North Carolina.

Brad Baker currently serves as the EMS Assistant Chief of Training and Clinical Affairs at Johnston County Emergency Services EMS Division. He brings with him over 14 years of experience in the field of EMS and fire services. Brad graduated from Western Carolina University's emergency medical care program and is an adjunct instructor at Johnston Community College where he teaches EMT courses. 

Brad has an unwavering passion for education, especially to support the growth of prehospital providers and community members. His team comprises of the agency's training officers and their community paramedic program. Over the past five years, Brad has had an exceptional opportunity to build his team, resulting in a high-performing and successful team. 

Brandon Miller is a Captain with Gaston County EMS in Gastonia, North Carolina. With over 10 years of experience in emergency medical services, he has been instrumental in developing the Community Paramedic program, which addresses the physical, mental, and emotional needs of community members. In addition, he has led several initiatives, including a post-overdose response team, a medication-assisted treatment (MAT) bridge program, a recovery transportation program, and the implementation of a jail-based MAT program. Brandon holds a Bachelor of Science in Emergency Medical Care from Western Carolina University.

Brian Mahaffey MBA, RN, NRP, CCP-C is the Captain/ Coordinator
over the McDowell County EMS Community Care Paramedic Program.
Brian has been a Nationally Registered Paramedic for over 23 years and
an RN for over 12 years. He has held positions in EMS including EMS
Supervisor, Medical Training Officer, Tactical Medic and Flight Medic. Brian
has worked as an RN in the Emergency department of several hospitals
and was a Trauma Lead Nurse at a level I Trauma Center for several years
as well as serving as a career Fire Captain for a large, combined EMS and
Fire/ Rescue Department in South Carolina.

Brianna Jones is the coordinator for the EMS Prevention Program for Tuscaloosa Fire Rescue in Tuscaloosa, AL. Brianna provides day-to-day supervision of the EMS Prevention program. Brianna’s background is in medical social work and case management. She graduated from the University of Alabama with a master’s in social work in 2017. Brianna has been working as the coordinator for EMS Prevention since 2019. She was also a student intern for the program while pursuing her master’s degree in the spring of 2017.

As the Human Service Counselor with Marion Senior Services, Dr. Kelley connects with the Senior community to identify potential client needs, services and gaps in services, provide geriatric counseling, and educate caregivers on maneuvering coordination of services and advocacy for their loved ones. As a scholar-practitioner, Dr. Kelley is committed to continually learning and growing to educate Seniors and Caregivers on planning and maintaining a safe, fulfilling, independent lifestyle for as long as possible.

Dr. Kelley earned her doctoral degree in Human Services from Walden University in 2020. She has over seventeen years of experience with crisis intervention, mitigation, and research in stressors and coping in emergency services. She has researched 9-1-1 processes, planned and presented local, state, national, and international conference in the emergency service realm, and been a Law Enforcement Clinical Responder, responding with law enforcement to individuals actively in crisis. She also holds a Master of Science in Mental Health Counseling from Walden University and a Bachelor of Arts in Anthropology from the University of Florida.

Dr. Kelley recognizes that society evolves at an ever-increasing rate and has a deep desire to be part of making a positive social change in her community. Her current projects include organizing multi-agency agreements to identify ways to improve the continuum of care, jail and Baker Act diversions of Seniors, geriatric counseling, and caregiver support.

Chris Hickman serves as an EMS Captain for Ocala Fire Rescue, where he has been employed since 2001. He is a board-certified Community Paramedic with nearly 30 years of experience. Leading the chronic illness division of the Community Paramedicine program, Chris has partnered with Marion Senior Services to develop and provide advocacy for vulnerable seniors. He holds an associate degree in Fire Science and has received several honors throughout his career, including:

· Unit Citation for rescuing victims in 2001

· Fire Service Medal of Valor in 2008

· Liberty Mutual Award

· Firefighter of the Year

· Driver of the Year

· Hometown Hero Award in 2019

Chris is also an active team member of the local CoResponse Team, overseeing the medical portion of the care provided. Through active advocacy and essential discussions with community leaders, he helps bridge gaps in medical education, resources, planning, and support.

Chris Holloway has dedicated over 28 years to Tuscaloosa Fire Rescue, serving in various vital roles such as paramedic, firefighter, company officer, and station captain. For the past decade, he has excelled in his current role as EMS Chief. He was instrumental in launching the EMS Prevention Program in 2014, demonstrating his commitment to community safety and well-being. Chris has been married to his wife, Holly, for 26 years, and they have one daughter, Anna, who is a Junior at Liberty University. Outside of his professional achievements, Chris is active in his church and volunteers at the local Boy Scout council.

Christopher Goenner, originally from Pennsylvania, brings over 34 years of experience in public safety to his role as a speaker. He began his career as a volunteer firefighter, EMT, and law enforcement officer before dedicating a decade to full-time EMS education. After relocating to North Carolina in 2007, he earned both his North Carolina and National Registry Paramedic certifications. Chris holds an Associate in Applied Sciences in EMS from Lenoir Community College, a Bachelor of Science in Emergency and Disaster Management, and a Master's in Health Sciences in EMS Education from Western Carolina University.

Chris has served as a field training officer at Cabarrus County EMS and recently held the position of Department Chair of Emergency Medical Sciences at Central Piedmont Community College. He is an active presenter at local, state, and national EMS conferences, has published research, and received a national award for his work on End-Tidal Carbon Dioxide in septic patients. A former president of the North Carolina EMS Educators Association, he participates in several committees with the North Carolina Office of EMS and has taught as adjunct faculty at Rowan-Cabarrus Community College, Stanly Community College, and the University of North Carolina at Charlotte. He is currently the Program Chair and Associate Professor for the Bachelor of Science program at Cabarrus College of Health Sciences.

With 25 years of experience in medicine, Christopher Rejowski has built a diverse and accomplished career. He began his journey as a New York City Paramedic, serving for six years before transitioning to a Physician Assistant role, where he has dedicated the past 20 years to Emergency Medicine, General Surgery, and Hospital Based Medicine. 

Currently, Christopher works with Atrium Health Hospital at Home, in Charotte, North Carolina,  where he leverages his extensive clinical background to enhance patient care. He previously served as a Clinical Advisor for the Metrolina Healthcare Preparedness Coalition, gaining valuable insights into healthcare readiness and emergency management.

Christopher is currently pursuing a master's degree in Emergency and Disaster Management from Thomas Jefferson University. His unique blend of clinical experience and academic pursuit allows him to bring a distinctive perspective on clinician-based emergency management to the healthcare setting, ensuring that preparedness and response are integral components of patient care.

Christopher Warr is an accomplished healthcare leader with two decades of experience in emergency medical services (EMS), critical care, and paramedic education. As the Critical Care Paramedic Educator and Manager of AirCare and Mobile Integrated Health at Atrium Health Wake Forest Baptist Medical Center, Christopher has been instrumental in expanding paramedic roles beyond traditional prehospital care.

With certifications as a Flight Paramedic-Certified (FP-C) and Community Paramedic-Certified (CP-C), his clinical experience spans both ground and air medical transport, emergency department operations, and innovative in-home care models. Christopher was part of the first cohort of paramedics to function in non-traditional roles within the Medical Center, where he has helped shape programs like Hospital@Home, which was developed to address system capacity challenges during the COVID-19 pandemic. This model has since been adopted across multiple systems within the Atrium Health network.

As a North Carolina EMS Level II Paramedic Instructor and adjunct faculty at both Wake Forest University and Rowan Cabarrus Community College, Christopher has dedicated his career to advancing the education and clinical readiness of EMS professionals. He has led programs that have consistently produced high-performing paramedics, enhancing their skills in critical care and emergency medicine.

Throughout his career, Christopher has combined his passion for education, clinical expertise, and leadership to drive forward patient-centered initiatives and expand the role of paramedics within integrated healthcare teams. His contributions are helping to shape the future of EMS, ensuring that paramedics are recognized as essential providers in evolving healthcare models.

Colin Johnson serves as a Deputy Chief in the Office of the Medical Director within the Lee County Department of Public Safety, Division of Emergency Medical Services. With over 15 years of experience in EMS, Colin's role encompasses a diverse range of responsibilities including clinical quality assurance, improvement initiatives, patient safety, protocol development, procurement of medical supplies, educational initiatives, and the identification of clinical best practices.

Prior to assuming his current position, Colin held various roles within Lee County Emergency Medical Services, including Division Chief, Captain, Lieutenant, Field Training Officer, Paramedic, and EMT. His educational background includes a Master of Science Degree in Psychology and a Bachelor of Science Degree in Health Studies.

Courtney Nelson is a Nurse Manager in the Southeast Region of Advocate Health and oversees the Primary Care Connect and Annual Wellness Visit Nurse teams. She leads the team's abilities to deliver comprehensive patient care, improve patient access, and enhance preventive health initiatives. The team’s partnership with Mobile Integrated Health (MIH) has led to reduced healthcare costs, improved health outcomes, and increased patient satisfaction by having options to deliver care in the home. Since joining the Primary Care Connect team in 2021, Courtney has expanded the team’s support for outpatient clinics across four markets. In 2023, she assumed leadership of the Annual Wellness Visit Nurse team and is actively working to extend this program throughout the Southeast.

With over a decade of experience as a nursing leader within Atrium Health's medical group, Courtney possesses extensive knowledge in family medicine, internal medicine, and pediatrics. Her previous roles include serving as a regional patient safety coordinator and as a clinical supervisor for the high-risk postpartum unit at Atrium Health Carolinas Medical Center.

Courtney is currently pursuing her Doctor of Nursing Practice (DNP) in Executive Healthcare Leadership at the University of South Carolina. She holds a master’s degree in nursing leadership from Cabarrus College of Health Sciences and is board certified in Ambulatory Care.

Cindy Irving started her career in Law Enforcement at the Marion County Sheriff’s Office as a Law Dispatcher/TTY Operator/911Telecommunicator in January of 2000. In 2005, Cindy became a victim advocate, and in 2008 became a Crisis Intervention Specialist. On April 20, 2018, appointed by Sheriff Billy Woods, Cindy helped develop the Juvenile Intervention Specialist position after the Forest High School shooting. In October of 2023, Cindy Irving became the Senior Intervention Specialist with the Marion County Sheriff’s Office and a part of the Mobile Integrated Health Team.

Cindy is a Victim Practitioner through the Office of the Attorney General and has been working with victims of crime for the past nineteen years. She has been teaching the Personal Awareness/Self Defense classes since becoming Victim Advocate in 2005. In 2018, Cindy became a certified Rape Aggressive Defense (RAD) Instructor. She has recently become a Peer Support Team member to assist coworkers in different circumstances.

Cindy has an AA in Psychology from Central Florida Community College, a BA in Psychology from the University of Central Florida, and a Masters in Social Work from Simmons College. Cindy has served as co-chair on the Marion County Children’s Alliance Family Violence Prevention Workgroup. Cindy currently serves on the Baker Act Task Force, Continuum of Care, Public Policy Institute, Local Review Teams for juveniles who need services, and other committees. Cindy was the 2012 Marion County Children’s Alliance Family Violence Workgroup’s Victim Advocate of the year, was selected by her peers as Marion County Sheriff’s Office 2013 and 2021 Humanitarian of the Year, and in 2021 received the Marion County Children’s Alliance Family Violence Prevention Workgroup Conqueror Award.

Dawn has been with Advocate Health Midwest Region for 28 years, and her experience spans multiple programs within Continuing Health. She has operational experience in home health services, hospice and provider-based programs, including the Post Acute Network Skilled Nursing Facility Care Program and Advanced Care at Home, which is comprised of Palliative Care at Home, Physicians at Home and Complex Care at Home. Prior to joining the organization, she held previous roles in education, quality and regulatory issues. In her current role, she leads the value-based initiatives for Continuing Health across the enterprise, working closely with Population Health, Care Management, Utilization Management and hospital leadership. As a member of the Moving Health Home coalition, she works closely with government affairs to assure that advocacy work addresses the mission and goals of Continuing Health.

Deidre Rolli, a seasoned healthcare professional, serves as a Clinical Consultant for Current Health/Best Buy Health, bringing over 25 years of invaluable experience in Emergency Medicine. She joined the team in October 2022 as a Subject Matter Expert in the Hospital at Home/Remote Patient Monitoring (RPM) space, transitioning from her impactful role at UMassMemorial Healthcare.

Deidre's extensive career has been marked by her dedication to patient care at both the bedside and in leadership positions. As the former nurse manager, she played a pivotal role in the establishment and expansion of the Hospital at Home Program. Her expertise spans patient flow optimization, care coordination, and the enhancement of the athome care experience.

A true innovator, Deidre is passionate about leveraging technology to transform healthcare delivery. Her deep understanding of Emergency Medicine, coupled with her leadership in implementing Hospital at Home initiatives, positions her as a subject matter expert in the dynamic intersection of healthcare and technology.

In her current role, Deidre continues to drive advancements in remote patient monitoring, contributing to the evolution of healthcare delivery beyond traditional hospital settings. Her commitment to improving patient outcomes and experience remains unwavering, making her a valuable asset to the Current Health/Best Buy Health team.

Dennis Parker, EMT-P/Engineer is currently with Noblesville Fire Department in Noblesville, Indiana for 25 years. Dennis current role is a Community Resource Paramedic (CRP) with Noble ACT as a collaboration between the Fire Department and Police Department with a common goals to support and provide resources to community members. Together they work to decrease 911 calls for Mental Health, Substance Abuse, Homelessness and MIH. Noble ACT utilizes 3 Certified Therapy K9's for Crisis and Follow up after the crisis, this resource has helped to open conversations that would have been in the past refused from clients. Dennis' 34 year career includes working both 911 and Private in areas of small Rural and large City of Indianapolis 911.

Emily Castor is the Mobile Integrated Health Coordinator with Indiana Department of Homeland Security, EMS Division. She coordinates and facilitates MIH EMS for the state of Indiana and has an extensive hospital and Outpatient Services background with experience in Oncology, inpatient and outpatient Lab Services, Urology, General Surgery and Trauma.  Emily is dedicated to continuing the growth of MIH and Community Paramedicine in Indiana with hopes of making services available to all Hoosiers.  

 

Eric Chaney is a results-driven Emergency Medical Services (EMS) Program Manager with over 35 years of experience developing and interpreting regulatory requirements, laws, doctrine, policy, and program guidance for local, state, and federal fire & emergency services (F&ES) agencies. In his current role as an EMS Specialist with the Department of Transportation, he serves as the Project Manager for the National EMS Information System (NEMSIS).

Prior to his current position Mr. Chaney held the position of Department of Homeland Security (DHS), Office of Health Affairs Branch Chief for the Medical First Responder Coordination Branch. Eric has worked as an EMS Analyst with Commander, Navy Installations Command (CNIC) F&ES, where he developed, formulated, and integrated plans, policies, and procedures necessary for effective execution of the U.S. Navy CNIC F&ES Program protecting military personnel, their dependents, and civilians on Navy installations worldwide.

Eric holds a Master of Business Administration Degree from Mount Saint Mary’s College, a Master of Science Degree in Emergency Health Services from University of Maryland Baltimore County and a Paramedic certification from the National Registry of Emergency Medical Technicians.

Erik J. Blutinger, MD, MSc, FACEP is a full-time emergency attending physician at Mount Sinai Health System in New York City. As Medical Director, he has overseen Sinai's Community Paramedicine program for years, building new clinical protocols, forging new partnerships, and training paramedics and physicians alike. Erik has worked in different healthcare systems around the globe, serving underserved communities in places like South Africa and Guatemala. He has written peer-reviewed journal articles, advised start-up companies, presented at major conferences, and has been recruited by global media networks for building safe and effective telemedicine & home care medical solutions for patients and their communities.

Felisha Richards, MBA is a Senior Strategist at Community Asset Builders who is responsible for developing innovative solutions while interfacing with diverse stakeholders to support sustainability of programs, organizations, and communities. She has over thirteen years of combined experience within the behavioral health, healthcare, and higher education fields. She is passionate about the advancement of emerging promising practices, specifically in areas of substance misuse, overdose prevention, mobile integrated healthcare, homelessness, maternal health, and rural. She fosters discussion, leads activities, and encourages partners to dive deeper into challenging and complex topics for resource development, program management, and evaluation. She holds a Master of Business Administration and resides in Rolla, Missouri.

The Olathe Fire Department Mobile Integrated Healthcare team connects patients with the right care in the right place at the right time. We have two teams, each made up of a firefighter paramedic and nurse practitioner. Our program is a three-way partnership with our local hospital, our federally qualified health center, and our fire department. We provide primary care services to Olathe community members at no cost to them, wherever they need it. These services also include referrals to behavioral health services and substance use treatment.

Greta Beach, RN, BSN, has worked for Spartanburg Regional Healthcare System for more than 6 years. After graduating from Clemson University in 2008 with a degree in Spanish and International Health, she started her career as the Executive Director of the Good Samaritan Clinic in Garden City, GA, which is part of St. Joseph’s/Candler Health System. After working in various non-clinical roles, she shifted careers and graduated with her bachelor's degree in Nursing from Clemson University in 2017. In 2018 she began working for Spartanburg Regional in the Cardiac ICU. In 2023 she transitioned to the Heart Failure

Clinic as the Patient Outreach Nurse. In this position, she has been able to use her experience in public health and cardiac nursing to address the social determinates of health affecting heart failure patients.

Dr. Augustine currently serves as the Medical Director for Lee County EMS in Fort Myers.  He is also in Medical Director roles for North Collier Fire Rescue in Naples, Florida, the City of Fort Myers Fire Department, the Lehigh Acres Fire Control District, and Washington Township Fire Rescue near Dayton, Ohio.

Jim began his career in EMS when he was in medical school in Dayton.  He became a firefighter in 1982, then began his service as a Fire EMS medical director in the Dayton area in 1987.  After serving as the President of the regional Fire EMS council, he became the first Chair of the Ohio EMS Board.   He moved to Atlanta in 2001, and served as Medical Director for Atlanta Fire Rescue, including the Hartsfield Jackson Atlanta International Airport Fire Rescue division.  He later served as Assistant Fire Chief and Medical Director for the District of Columbia Fire EMS Department.  While there, he also served as Medical Director for the Metropolitan Police Department. He is a member of the National Association of EMS Physicians (NAEMSP), and is a key member of the “Eagles” group of Fire EMS Medical Directors. His current roles include the responsibility of serving as Medical Director for the International Association of Fire Chiefs (IAFC).

James McLaughlin  is the Director of Community Paramedicine with Ute Pass Regional Health Service District (UPRHSD) and is affectionately known by most within the community as “Tiny.”  James has been involved with emergency medicine since 1991 and has been honored to serve the citizens of Ute Pass Regional Health Service District since 2008.  James has a Bachelor of Science in Business Administration from Colorado Tech., focusing on International Finance.  The UPRHSD Paramedic Advanced Care Team (PACT) that James leads seeks to maximize value to the community by leveraging underutilized resources to meet the needs of marginalized and underserved populations.  James is trained in Trauma-Informed Care, Motivational Interviewing (MI), and Crisis Intervention Team (CIT) interventions and is a certified level-three Crisis and Hostage Negotiator. In 2021, James was named the Colorado State Community Paramedic of the Year.  James currently serves as chairman of the Pikes Peak State College Emergency Medical Service Advisory Committee, is a member of the Colorado Medical Board and the Colorado Mental Health Advisory Board, and is a Board Member of the Teller County Mental Health Alliance.  James is an adjunct faculty member for Pikes Peak State College and the prestigious University of Colorado Health System.

 I am the Director of Burke County EMS. I am new in the role and came from a large healthcare system that had a very established MIH program. I’m proud to serve as the Director of Burke County EMS. My journey began in 911 then moved into the healthcare system, where I had the opportunity to advance through critical care and into leadership. While in leadership I was able to help establish and support a Mobile Integrated Health (MIH) program. This experience fueled my passion for community health, and now I’m excited to bring that commitment to Burke County, where I hope to expand our single Community Paramedic resource and enhance our services.

In addition to my professional life, I’m a dedicated volunteer firefighter and first responder. This role has deepened my understanding of emergency services and the critical impact they have on our community. Outside of work, I’m a devoted father to three sons and have been happily married for 25 years. Family is my cornerstone, and I cherish every moment spent with them. I have a deep love for the outdoors and wilderness medicine, which not only inspires my personal adventures but also informs my advocacy for health education in natural settings. I believe that fostering healthy communities is essential, and I’m eager to strengthen partnerships and innovate approaches to emergency medical services in Burke County. Together, we can reduce healthcare gaps for all.

Jamesha Edwards is the CONECT Program Case Manager with James City County Department of Social Services. In 2020, Jamesha began her career as an Intake Specialist and was promoted to CONECT Program Case Manager in 2021. She focuses on prevention services, mobile-integrated health care, and adult service programs. She has a passion for helping people and advocating for them to reach their full potential. Jamesha graduated from Old Dominion University after obtaining her Bachelor of Science degree in 2016 majoring in Sociology. Jamesha enjoys spending time with her husband and two young children.

Jamie Pristasch, MSW, LSW is currently an Emergency Response Social Worker with Westfield Fire Department in Westfield, Indiana. The Emergency Response Social Worker is a partner to the Field Resource Paramedic within the MIH program in Westfield. A graduate of Indiana University School of Social Work at IUPUI, where she obtained her undergraduate and graduate degrees in Social Work. Studies for these degrees led to a focus on child welfare, substance use disorder, mental health and case management. Various lived experiences, personally and professionally have led to a true passion to help and serve people when they are most vulnerable. Prior to working at Westfield Fire Department, Jamie was employed as a Medical Social Worker at IU Health for over 9 years. Other professional experience includes Home Health Social Work and Department of Child Services.

Jay Fraccola is the Operations Manager for Atrium Health’s Mobile Integrated Health (MIH) program, the exclusive provider of in-home care for Atrium Health’s Acute Outpatient Services and Hospital at Home initiative. Over the past four years, Jay has been a key force in the program’s growth and innovation, helping to elevate patient-centered, home-based care. With extensive expertise in operations and healthcare management, Jay is deeply committed to advancing mobile health services and enhancing patient outcomes.

Jennifer Chester, MSN, RN, VA-BC, TCRN, is the Injury Prevention Coordinator at Atrium Health Cabarrus, where she focuses on advancing trauma care and injury prevention strategies. With over a decade of nursing experience, Jennifer has served in diverse roles across prehospital, emergency nursing, trauma care, and vascular access. She is board certified in both Vascular Access and Trauma Nursing (TCRN) and has played pivotal roles in leadership, education, and patient care. Jennifer holds a Master of Science in Nursing Administration from Lenoir Rhyne University and is active in various professional organizations, as well as regional and state Trauma Advisory Committees, and involvement in many local community groups.

Jennifer Correa has been with the NEMSIS Technical Assistance Center (TAC) for almost three years and is a Senior Business Data Analyst specializing in business and technical support. With more than 20 years of experience in data collection, the NEMSIS standard, NEMSIS standard technical requirements, and EMS data, Jen provides subject matter expertise to national data quality initiatives and research projects. She is a trainer and presenter at local, state, and national events. Jennifer works with agencies, states, vendors, and federal partners to improve EMS data collection across the industry.

Jeremey McKinney is a national speaker focused on leadership, leadership development and workplace culture. Jeremey holds a Bachelor of Arts in Biology and Religious Studies from Indiana University as well as credentials in ministry.  He became an EMT in 1998 and completed his Paramedic in 2003.  He has worked in 911, IFT and Critical Care markets, and has served as a Training Officer, Training Coordinator, Supervisor, Education Specialist, and Primary Instructor. Jeremey currently serves as the General Manager for Amerimed EMS overseeing operations in Indiana and Ohio.  He also serves as a member of the board for the Indiana EMS Association.  With over twenty-five years of leadership experience, Jeremey combines his knowledge of EMS and ministry to bring a new perspective on leadership development and workplace culture to healthcare

 Dr. Rowe is a Board-Certified EM Physician who is also fellowship trained and subspecialty board certified in Emergency Medical Services. As an EM/EMS physician he currently holds a clinical faculty appointment with the UF College of Medicine/UF Health Department of Emergency Medicine within the division of EMS. He is the Medical Director for Gilchrist County Fire Rescue, in Gilchrist County, Florida as well as the Co-Director for Lake Emergency Medical Services, Lake County, Florida. He has attending physician experience in both academic and community settings in multiple states. As a former academic chief resident, Medical Director of Emergency Departments, and Medical System Stroke Services Director he has interacted with multiple leaders in medical and administrative areas and developed experience in identifying effective teamwork and leadership. He is passionate about building the discipline of prehospital medicine knowledge base by addressing practitioner-identified problems of practice through community engagement, MIH, healthcare service delivery innovation and clinical service integration.

Jonathan Haner received his Bachelor’s Degree from Cornell University in 1990 and has been employed as a Paramedic since 1994. He began his career in the ski industry at Squaw Valley, California in 1991, working as a Ski Patroller. He worked as a Ranger during the summer months at Mesa Verde National Park until permanently moving to Pagosa Springs in 1996 and becoming a member of the Wolf Creek Ski Patrol. There he had many duties and responsibilities, both as a Paramedic as well as a Level III Avalanche Technician. He worked summers as a Paramedic with the Upper San Juan Hospital District and attained both Critical Care and Flight Paramedic certifications. He came to the Upper Pine River Fire Protection District in 2017 and has worked as a Paramedic Firefighter since that time. He has been deployed on multiple disaster assignments including wildland fires, hurricanes, and Covid-relief deployments. He has also been involved with the Upper Pine EMS Education Center, coordinating and teaching multiple EMT and NAEMT courses. He recently obtained his Community Paramedic endorsement and is working on implementing a program at the Upper Pine Fire Protection District

Jonathan Belott is currently a Fire Lieutenant-Paramedic and Mobile Integrated Healthcare Program Manager with the Milwaukee Fire Department (MFD).  He is responsible for guiding Mobile Integrated Healthcare (MIH) program operations and team members so they are best able to accomplish their mission of increasing public health within Milwaukee while reducing per-capita costs.   

Lieutenant Belott was appointed to the MFD as a firefighter in October 2011 before promoting to Heavy Equipment Operator and obtaining his Paramedic certification in September 2014. Lieutenant Belott joined the second class of Community Paramedic graduates from the training program provided by the School of Nursing at UW-Milwaukee in 2017.  

As the Community Paramedic program expanded, the MFD reopened a decommissioned firehouse to staff and house the expanding team.  In March 2019 Lieutenant Belott transferred to this newly repurposed firehouse to work full-time as a MIH Practitioner.  Within months of arriving there, Lt Belott was tasked with developing the new Milwaukee Overdose Response Initiative (MORI) and became the first MORI Supervisor.  He was promoted to Lieutenant in February 2020 and became the MIH-Supervisor later that year. Following the retirement of his predecessor, Lieutenant Belott assumed the role of MIH Program Manager. 

Lt Belott obtained a BFA in Music Education with a business minor from UW-Milwaukee in 2005.  He still actively plays tuba with various musical groups in the area.  He also enjoys taking his camper out with his family to Wisconsin state parks as often as he can throughout the nice weather months. 

Jordan Lobel holds a Master of Healthcare Administration from The Ohio State University. After completing his degree, he undertook an Administrative Fellowship at Atrium Health in the Greater Charlotte Market, where he reported directly to the market president.

Currently, Jordan serves as a Project Manager for the Hospital at Home Program, where he has led and contributed to various initiatives focused on business development and process improvement. He has a strong interest in strategy development, operational excellence, service line expansion, and healthcare innovation.

Outside of work, Jordan enjoys watching football with friends and family—he is a dedicated Packers and Ohio State fan—exploring the outdoors with his dog Gilly (Gil), traveling, engaging in community involvement and volunteer activities, and discovering new restaurants.

Josh has been in emergency medical services (EMS) for over 15 years. He earned a Master of Public Safety Leadership and Administration degree with a concentration in mobile integrated healthcare and is currently a Ph.D. candidate and active researcher. Josh is a board-certified community paramedic (CP-C) and an item writer/content expert for the International Board of Specialty Certification (IBSC) community paramedic board exam (CP-C). He is an adjunct professor of biological sciences. Josh is currently the Lead Community Paramedic Clinician at Monadnock Community Hospital and has been a part of the team since the Mobile Integrated Healthcare program started in April 2020, a program that he helped start and develop.

Justin Greer, EMT-P, CP-C, is a Certified Community Paramedic with 8 years of Emergency Room experience and 6 years of EMS experience. He has extensive expertise in opioid-related issues and has led initiatives in medication-assisted treatment, Naloxone distribution, and harm reduction.

Justin Lemery is the Chief of Emergency Medical Services for the Tulsa Fire Department. A second generation firefighter and a 17 year member, Chief Lemery leads the EMS Branch and the Mobile Integrated Healthcare Program. Chief Lemery is a nationally registered paramedic, and a community paramedic. Director Lemery is involved in various leadership positions that are committed to improving health and well-being such as: Community Health Improvement Plan leadership team, Community Health Council, International Co-Responder Affiliation (ICRA) board, National Association of Mobile Integrated Healthcare member, Region 7 Healthcare Coalition, Emergency Preparedness and Response Team Region 7, and Oklahoma State University-Fire Service Technology adjunct Professor.

I began my EMS career at Charlotte Motor Speedway as an EMT before joining Cabarrus EMS in 2012. I have earned a bachelor's degree from Johnson & Wales University in sports entertainment and event management. After completing the paramedic program at Rowan-Cabarrus Community College, I obtained my bachelor’s degree in Emergency Management and master's degree in Health Science from Western Carolina University. I have worked as a part-time instructor at Rowan-Cabarrus Community College and Central Piedmont Community College. I am a nationally registered paramedic. I am a credentialed instructor in numerous American Heart Association, National Association of EMT courses, as well as a North Carolina Level 2 Instructor.

Over the past twelve years with Cabarrus EMS, I have served as a track paramedic and community paramedic. I am now a Deputy Chief of Support Services which is the direct supervisor over our Community Paramedic division.

Karen has been in EMS for over 25 years with a diverse background in EMS Management, Communications, Education and Quality Assurance and Improvement. She holds degrees in Biology and Homeland Defense and Disaster Preparedness as well as several EMS Instructor certifications.

Karen has extensive experience in EMS Education as a State Certified Instructor Coordinator as well as an Advanced Life Support-level State Examiner.

Ms. Host joined Cataldo Ambulance Service in 2010 as the Quality Assurance and Regulatory Compliance Manager and has since advanced to her current role as Executive Director of Clinical Services/Mobile Integrated Healthcare.

Karen moved into a leadership role with Cataldo Ambulance Service’s launch of Mobile Integrated Healthcare – SmartCare - in 2014 when Cataldo became one of the two authorized pilot programs in Massachusetts. Karen then sat on the state MIH Advisory Council that shaped MIH regulations promulgated in 2019.

Karen has lectured across the country at national MIH conferences and has been recognized by Mass General/Brigham’s as a Pillar of Excellence Award recipient for MIH delivery of monoclonal antibody treatments to home-bound COVID positive patients. Karen is also an elected board member of the Massachusetts Region IV Medical Services Committee.

KATHERINE H. WEST, BSN, MSEd, has been working in the field of infection control since 1975. She lectures both nationally and internationally on this topic. She publishes books, video tapes and articles on infection control related issues. She has served as a consultant to the Centers for Disease Control and the National Institute of Occupational Safety & Health. She has served as an education specialist for the National Institutes of Health. Ms. West has recently authored, Infection Control Policies for Community Paramedicine & MIH.

Katherine has also served as a consultant to the US Public Health Service, Federal Occupational Health. Ms. West was voted a “Hero in Infection Control & Prevention” for 2006 for her work in the field of infection control & EMS. This honor was from the Association for Professionals in Infection Control & Epidemiology (APIC). Ms. West has also received the Virginia Governor’s Award for her contribution to EMS health & Safety and the James O. Page EMS Achievement Award (2020) from the International Association of Fire Chiefs, EMS Section.

She has been a speaker at many meetings and national conferences.

Keith Sharp is a dedicated firefighter with 22 years of service, currently serving as a Captain in the Community Resource Paramedic Office. In this role, Keith oversees the CORE (Coordinated Overdose Response by EMS) program, focusing on improving emergency responses to opioid overdoses and connecting individuals with life-saving resources.

With a degree in Fire Science and a commitment to lifelong learning, Keith is currently pursuing a Bachelor’s in Business Administration. His passion for both public safety and leadership drives his work in bridging emergency services with proactive community engagement.

Beyond his professional commitments, Keith is a proud husband and father of two, balancing family life with his mission to enhance public health and safety. Keith lives in Lake Stevens Washington where he and his wife also run a nonprofit helping vulnerable children.

Kevin is the Program Director for the National Emergency Medical Service Information System (NEMSIS). Prior to joining NEMSIS, he brought over 15 years of diverse EMS experience spanning fire service, private EMS, and third-service agencies. His extensive expertise includes roles as a field provider, ET3 administrator, and a highly regarded educator and trainer for Austin-Travis County EMS. Additionally, Kevin served as the Data Manager for the Office of the Chief Medical Officer in Central Texas, where he specialized in applying descriptive and predictive analytics to drive improvements in both patient outcomes and the quality of life for EMS providers. He holds an MS in Business Analytics and Data Science, with a focus on Advanced Data Science and Information Systems Management.

Kim Hendrickson is the Director of the City of Poulsbo’s Housing, Health and Human Services Department where she works on projects increasing access to healthcare, housing and social services. She oversees the City’s Recovery Resource Center, a friendly, low barrier hub for health and recovery services in North Kitsap County. Kim helped to develop the City of Poulsbo’s police navigator program, the Poulsbo Fire Department’s CARES program and Central Kitsap Fire and Rescue's CARES program. She encouraged the growth of co-response programs throughout the state through the Co-Responder Outreach Alliance (CROA) from 2018-2023. She currently works with the University of Washington’s BHCORE Center to promote fire department-based behavioral health initiatives.

Krista Ott was born and raised in Gainesville, Florida. She served as the full time Fire and Life Safety Educator for Gainesville Fire Rescue from 2006 to 2022. Since December of 2022, she has served as the Community Resource Paramedicine Program Coordinator. Her credentials include Fire and Life Safety Educator 2, Fire Inspector, Firefighter 1, EMT, Fire Service Instructor III, Youth Firesetter Intervention Specialist, CPR Instructor, and Child Passenger Safety Technician Instructor.

Krista currently serves as a student advisor for the University of Florida Fire and Emergency Services Bachelors Program and she serves on the Alachua County Healthcare Advisory Board. She sits on the Florida Occupant Protection Coalition and the Alachua County Community Traffic Safety Team.

Krista earned her bachelors in Fire and Emergency Services and her Masters in Emergency Management and Disaster Preparedness from the University of Florida.

In her spare time, Krista loves spending time camping, hiking, and fishing with her son William.

Kristen Hanks is the Registered Nurse Supervisor of the Primary Care Connect virtual team in the Southeast Region of Advocate Health. In this role, she helps lead patients through the care continuum by supporting Primary Care providers, specialty and inpatient partners, while collaborating with key partners like Mobile Integrated Health (MIH). Kristen is a founding nurse of the Primary Care Connect team, instrumental in expanding its reach from Greater Charlotte to the Floyd, Navicent, and Wake markets over the past three years. Her innovative collaboration with MIH has significantly enhanced patient care during transitions from higher levels of care, resulting in improved patient satisfaction, reduced hospitalizations, and decreased emergency department visits.

Previously, Kristen was a team lead for the COVID virtual hospital and has extensive experience as an emergency room nurse and in outpatient triage, where she identified critical needs for paramedicine in the community. She has also contributed to the development of Cognitive Impairment care pathways for her organization.

A dedicated professional, Kristen is a member of the American Nurses Association and holds board certification in Ambulatory Care nursing. She is currently pursuing her Master’s degree in Nursing Leadership at Cabarrus College of Health Sciences, where she continues to advance her expertise and commitment to nursing excellence.

Kristle Butler, NREMT-P, CP-C is the Community Paramedic for the Heart Failure Clinic at Spartanburg Medical Center in Spartanburg, South Carolina. Kristle has been employed with SMC for 15 years and is the first community paramedic for the hospital system which covers five hospitals in three counties. The Heart Failure Clinic's community paramedicine program was started using a Duke Endowment Grant and due to the program's success, the hospital has continued the program since the grant cycle has ended. Kristle is solely based in the Heart Failure Clinic with the goal of seeing heart failure patients discharged from the hospital within 7 days. She has been seeing patients since October 2022 and has assisted in a drastic 70% reduction of emergency visits and admissions for heart failure patients enrolled in the program. Recently passing the 2-year anniversary of the program's inception, Kristle has enrolled 150 patients and performed 800 home visits.

Lauren Ramsey is the EMS Prevention Case Manager at Tuscaloosa Fire where she provides targeted case management to clients within the program. She is a licensed master’s level social worker and obtained her master’s degree from the University of Alabama in 2019. Lauren joined the EMS prevention team in 2022. She has previous experience working for child protective services at the Department of Human Resources within the foster care division.

A 25 year Fire/EMS veteran, Patrick is one of two Community Paramedics at the UVA Health System in central Virginia. With a diverse background in public safety, Patrick has spent time with UVA’s ground ALS program (Medic 5), air/ground neonatal specialty team (NETS), 9-1-1 telecommunications supervisor, and as the regional EMS Quality Assurance Officer. Patrick also serves as the Deputy Chief, Paramedic, and Life Member of the Western Albemarle Rescue Squad in Crozet, Va since 2007. Patrick also currently works part time with the UVA Special Event Medical Management team where he supervises and provides emergency care to large event attendees of sporting events, concerts, or other large gatherings. In 2022, he was hired in the first ever group of Community Paramedics at UVA and he helped set policies, procedures and designed what the program would look like and how it would operate. The UVA Community Paramedicine program has helped reduce emergency room utilization by 75% in the patients they work with since seeing it’s first patient in October of 2022.

Lisa Walker has been with the James City County Fire Department for almost 9 years. Over the past 3 years she has worked in our MIH Program CONECT and is currently serving as Captain of Community Health. Lisa is a graduate of Virginia Tech, where she earned a Bachelor’s of Science in Human Nutrition, Foods and Exercise in 2003. She enjoys the beach, working out and lives with her husband, twins and a small farm of animals in New Kent, Virginia.

Mandy McGowan is a Registered Nurse in the Madison, WI, area. She is the Director of Home-Based Care Programs for UW Health, including Home-Based Primary Care and Home-Based Hospital Care. Mandy has worked in the home health arena for 18+ years and has spent most of that time in various leadership roles, with a focus on integrating home-based care services into the larger health system continuum of care. Prior to home health, Mandy worked as a bedside nurse, charge nurse and flight care nurse in Neonatal ICUs in Madison, WI, and New Orleans, LA.

Dr. Bidwell is one of the 2024-2025 Carolinas Medical Center EMS Fellows. He completed Emergency Medicine Fellowship at the Memorial Healthcare System in Hollywood, FL. He completed his medical education and a Master's of Pubic Health at St. George's University. Prior to medical school he was a Paramedic through the Hudson Valley and Bronx, NY. Has a passion for Mobile Integrated Health seeing it as the crossroads between EMS, Medical care and Public Health.

Megan is a dedicated professional who has been at the forefront of the expansion of mobile crisis services in her community. She is the Crisis Team Supervisor at Jackson County Mental Health in Medford, OR, where she has worked for the past 6 years. Since beginning her career in crisis work in 2013, Megan has cultivated a profound commitment to helping individuals navigate their most difficult times. She gradated with her master’s degree in Clinical Mental Health Counseling from Southern Oregon University in 2019 and is a Licensed Professional Counselor in Oregon.

Megan’s extensive experience and knowledge in the field enable her to lead effective response teams while providing compassionate support. She finds immense meaning and purpose in her work, striving to make a positive impact in the lives of those she serves. Her approach to crisis work is deeply rooted in a team-based approach and she believes that connection in itself is a healing force.

Mike Campbell is the Training Division Chief for Stanly County EMS. Part of this role entails supervision of the SCEMS Community Paramedic Division. Chief Campbell played a vital role in the development and implementation of the Community Paramedic Division, which was incepted in 2018. Mike has been in EMS since 2006 in various roles and positions including both volunteer and professional positions. Chief Campbell is also a NCOEMS Level 1 Paramedic Instructor and values education and understands a large part of working in healthcare is understanding that once you enter the field you become a life long learner. Mike was the former Co-Chairman of then North Carolina Community Paramedic Coalition, is a member of the National Association of Mobile Integrated Healthcare Providers, a member of the North Carolina Association of EMS Administrators, and the North Carolina Association of EMS Educators. Through his various roles, Mike has a passion for expanding and improving the type and quality of healthcare that Community Paramedics in North Carolina are able to deliver. Mike is happily married to his best friend, Amber and has 2 amazing, bright, and exceptional daughters, Ada and Abby. Mike is thrilled that his oldest is following in Dad’s footsteps and is starting EMT-Basic training in November. In his free time, Mike enjoys spending time with his family, being outdoors, and playing with his two dogs, Charlotte and Bonnie.

Myechia Hudson, MSW is currently in the role of a Social Worker/ Crisis Intervention Specialist with Noblesville Police and Fire Dept, in Noblesville, Indiana. The Social Worker/crisis Intervention Specialist is partnered with the Community Resource Paramedic with the MIH program in Noblesville. A graduate of Indiana University-Purdue University Indianapolis School of Social Work, where she obtained an undergraduate and graduate degree in in Social Work. The Social Work degree has allowed her the opportunity to work in a variety of diverse settings such as with children and families, mental health/ addiction and as a medical social worker. Myechia is very passionate about helping and serving those who are underserved and most vulnerable.

Captain Nicole Picknell has 30+ years Fire Ground experienceincluding wildland and structural firefighting, 25 years as a Paramedic. She has spent the last 5 years overseeing the Community Resource Paramedic Team at South Snohomish Fire and Rescue Regional Fire Authority (South County Fire). The South County Fire CRP Team has grown over the last 5 years from 2 CRP’s and 1 Captain to a robust team of 15 including 24/7 Community Resource Paramedics, Falls and AFH/SNF division and Community Health Worker services.

Nicole’s vision is to support community needs and the fire crew needs by finding the right service, at the right time, in the field and meeting the client where they are at. Her goal is to have a County Wide Community Resource Task Force that provides CRP resources to all surrounding agencies.

She has a huge desire to support Firefighter, Police Officers and first responders in health and wellness. She supports people in health and nutrition as a personal trainer, exercise instructor, nutrition coach, body builder coach, yoga instructor, Yoga for First Responder instructor and But yoga instructor.

Olyvia Wood has served as a Social Worker with the Department of Mobile Integrated Health since October 2021. She obtained her bachelor's degree in social work and psychology from Indiana State University in 2014. In addition, she earned a master's degree in social work from Louisiana State University in 2019 and has accumulated a decade of experience in the field. Her professional background includes work with children and families through the Department of Child Services, the judicial system, investigations of child fatalities, collaboration among various agencies, and, presently, addressing issues related to substance use and significant mental health disorders. In 2025, she obtained her social worker license, further solidifying her commitment to the profession and enhancing her ability to serve individuals in need

A 25 year Fire/EMS veteran, Patrick is one of two Community Paramedics at the UVA Health System in central Virginia. With a diverse background in public safety, Patrick has spent time with UVA’s ground ALS program (Medic 5), air/ground neonatal specialty team (NETS), 9-1-1 telecommunications supervisor, and as the regional EMS Quality Assurance Officer. Patrick also serves as the Deputy Chief, Paramedic, and Life Member of the Western Albemarle Rescue Squad in Crozet, Va since 2007. Patrick also currently works part time with the UVA Special Event Medical Management team where he supervises and provides emergency care to large event attendees of sporting events, concerts, or other large gatherings. In 2022, he was hired in the first ever group of Community Paramedics at UVA and he helped set policies, procedures and designed what the program would look like and how it would operate. The UVA Community Paramedicine program has helped reduce emergency room utilization by 75% in the patients they work with since seeing it’s first patient in October of 2022.

Ray Antonacci, BS, EMT-P has been the Chief for Lincoln County Ambulance District for over 10 years and began his EMS career in 1992. Ray has served on the Missouri East Central EMS Committee, Missouri Ambulance Association, and the Ambulance District Association of Missouri. Ray is an Adjunct instructor for Lindenwood University’s paramedicine program. He has served the underserved and historically under-resourced populations in many capacities throughout his career.  

Robert has worked in EMS since 2005. He has been a paramedic in eight states around the country and served as a program manager at the U.S. State Department; he founded the Mobile Integrated Healthcare program at McGregor Memorial EMS in 2021. Robert has a passion for improving people's lives through innovative programs that expand access to care by overcoming obstacles rooted in the social determinants of health. A board-certified Community Paramedic, Robert is also an award-winning educator. He holds a BA from Dartmouth College and an MPH from The George Washington University.

Dr. Constantine is an Emergency Medicine and EMS Physician in Charlotte, North Carolina. He is the Medical Director for Atrium Health MIH, EMS Fellowship Program Director at Carolinas Medical Center, and Medical Director for Stanly County EMS in Stanly County, NC. He is an Assistant Professor of Emergency Medicine for Wake Forest University School of Medicine. His interests include maximizing quality in out-of-hospital care in acute settings and innovations in out-of-hospital care delivery models.

Sabrina is the driving force behind Mercy Flights' Mobile Integrated Healthcare (MIH) program. With over a decade of experience, she has been at the forefront of the MIH movement at Mercy Flights since its inception in 2016. She is a certified Community Paramedic and CHW and the Chair of the Oregon MIH Coalition, which advocates for MIH work throughout the state of Oregon.

Sabrina's passion for MIH is contagious. She is committed to promoting growth and collaboration within her community and beyond. Her extensive knowledge and expertise have made her a renowned voice in the MIH community, locally and internationally.

When she's not busy revolutionizing healthcare, she loves to spend time with her family and exploring the beautiful landscapes of Southern Oregon with her dogs. She is also an avid gardener and enjoys tending to her plants in her free time.

Samantha Mitchell leads the City of Crawfordsville's Mobile Integrated Health Department as its Director. A committed member of the Crawfordsville community, Samantha returned to her hometown after earning a Bachelor of Science degree in Public Health from Purdue University in 2014. She began her career at the Montgomery County Health Department, focusing on public health education and achieving departmental accreditation. In 2021, Samantha joined the Crawfordsville Fire Department, where she significantly expanded Mobile Integrated Health programs and utilized data to improve their outcomes. She became the Director of the Mobile Integrated Health Department in January 2024, when the mayor established the program as an official city department. Samantha actively contributes to her community through her involvement in various boards and coalitions dedicated to addressing Montgomery County’s top health priorities

Sarah Czarnecki  RN,BSN, EMT-P, FP-C is a Paramedic at Lincoln County Ambulance District and has served for over 22 years in EMS.  She has worked with the underserved and historically under-resourced populations in Lincoln County for her entire career therefore she has an extensive familiarity with the culture and language of the population of focus. Sarah has recently taken on the role of MIH Liaison and has organized the program in teams to meet the needs of the citizens.

Sarah Piper serves as the Community Paramedic Coordinator for Johnston County Emergency Services in North Carolina. With close to two decades of experience in EMS, Sarah is deeply dedicated to Mobile Integrated Healthcare. The Community Paramedic Program, established in 2015, has evolved to address the needs of both the aging and pediatric populations. Sarah and her team have recently laid the groundwork for a post-overdose response team. As a parent herself, Sarah recognizes the importance of ensuring the provision of optimal healthcare to every member of the community, regardless of their age.

Dr. Scott Cariello is a Clinical Pharmacist and a double board-certified Paramedic in Critical Care and Community Paramedicine. He received his Bachelor of Science in Pharmacology and Toxicology and Doctorate of Pharmacy from Philadelphia College of Pharmacy, University of the Sciences in Philadelphia and his Associate in Applied Science, Emergency Response and Operations from Mesa Community College.

He currently works as a Paramedic with Maricopa Ambulance, located in Phoenix, Arizona. Scott also consults for multiple Health Plans and Health-Tech startups. Dr. Cariello is also an intermittent Federal employee with a Disaster Medical Assistance Team based out of Pennsylvania under the Department of Health and Human Services, Administration for Strategic Preparedness and Response, Office of Emergency Management.

Scott is passionate about supporting underserved populations and expanding healthcare access to all. He currently splits his time between Mesa, Arizona and Houston, Texas, and enjoys spending his free time with friends and family, outdoor activities, and staying cool during the Arizona & Texas summers.

Dr. Scott Goldberg, MD, is an emergency medical specialist in Boston, Massachusetts. He attended and graduated from Tufts University School of Medicine in 2008, having over 16 years of diverse experience, especially in Emergency Medicine. He is affiliated with many hospitals including Brigham and Women's Hospital.

Dr. Goldberg also received a MPH at the University of Texas and has maintained a strong interest in further public healthcare.

In addition to several board positions, Dr. Goldberg currently serves as the primary Medical Director for SmartCare, Cataldo Ambulance Service’s MIH program overseeing training, policies and procedures and continuous quality improvement.

Sean Lyons is an Educator, Innovator, and Communicator who's focus on patient centered care has spanned through his career in rural Fire and EMS,  Hospital-based Paramedic and CP teams, and as a national MIH leader in corporate health care enablement. Having built and managed programs designed to improve patient and caregiver satisfaction, improving patients access to care and healthcare equity, and promoting the Paramedics role within existing and innovative Home-based care models. Sean has led multidisciplinary teams to provide Primary care, Post Acute Transitional Care, Urgent and ED care, and Acute Hospital care at home. 

Shane Hardwick has 30 years of street-level paramedic experience, which includes former EMS Chief, SWAT/EOD medic, a DT4EMS instructor, and community paramedic serving in Indianapolis. Shane is a member of Shepherd Community Center’s Shalom Project.

Shelby VanDerMoere is the Mobile Integrated Health Program Manager and Community EMT for the City of Bloomington Fire Department. Shelby began her journey with BFD in October 2022 where she provides gap care for patients before and after 911 calls. With a Bachelor of Science in Human Services from Purdue University Global, an Associate of General Studies in General Studies, a certification for a Community Health Worker, and as a nationally registered EMT, Shelby is uniquely positioned to provide holistic care to the community in their place of need. 

Previously, Shelby served as the victim’s advocate for a prosecutor’s office for five years where she honed her skills in advocacy, ultimately preparing her for the patient advocate work she does today.  She serves on the Commission on the Status of Children and Youth for the City of Bloomington where she has been able to carry on her passion for creating and supporting healthy families.

This past year, Shelby was the recipient of the 2023 Indiana EMS Speciality Care Provider of The Year award presented by the Indiana Fire Chiefs Association, and her team was given an Innovation Award for “Most Progressive Public Safety Innovation” by the City of Bloomington. Most recently in 2024, Shelby was named a Mental Health Champion for Monroe County Nominee and the 2024 Outstanding Caregiver by The Community Dementia Alliance of Bloomington and IU Health Foundation, ADRS. 

When Shelby takes off her professional hat, you will likely find her wearing some sort of sports mom gear cheering on her kiddos. She has a beautiful blended family with 4 children and a husband that serves right alongside her. Shelby enjoys baking, podcasts, and jeep rides.

Taylor is a Crozet, Virginia native through and through. Taylor has been in EMS since 2011 in various capacities, starting at the Western Albemarle Rescue Squad. Taylor is a Medic, Captain, and lifetime member of the esteemed Western Albemarle Rescue Squad within Albemarle County Fire Rescue. His array of certifications, include, EMT-I, Swift Water Rescue, Emergency Vehicle Operator Class III, and Vehicle Extrication, Advanced Cardiovascular Life Support (ACLS) and Pediatric Advanced Life Support (PALS). Taylor currently holds an Associates in general studies from Piedmont Virginia Community College, and is slated to graduate with a bachelor’s degree in Health Sciences Management from the University of Virginia in the spring of 2025. Taylor previously worked as a full-time medic with the UVA Special Event Medical Management (SEMM) team starting in 2014. In 2019 Taylor transitioned into the UVA Population Health department in a remote role working with patients to decrease readmissions. In 2022 Taylor was one of two people in the Population Health Department that was moved into the Community Paramedicine role. Together Taylor and his partner developed the UVA Health, Community Paramedicine program from the ground up. Taylor has been seeing patients in this role for the last 2 years which touts the extremely encouraging data of a >70% decrease in ER visits. Taylor is beyond excited about this new opportunity in EMS and what it can mean for providers and patients alike. He is thrilled to share advice, experiences, and knowledge in this new field as the work is invaluable

Thomas Striegel, DO, FACEP is an Assistant Clinical Professor of Emergency Medicine at University of Iowa Hospitals and Clinics, Iowa City, Iowa where he is faculty for the Emergency Medicine Residency and the Carver College of Medicine. He was previously faculty at the University of Arizona College of Medicine in Phoenix where he served as Emergency Medicine Clinical Clerkship Co-Director. He is board certified in Emergency Medicine and Hospice/Palliative Medicine and aligns the two specialties with teaching and research in open-hearted communication, patient/physician bias affecting medical decision making and healing caregiver emotional trauma. Dr. Striegel also collaborates within the EMS Division, developing Mobile Integrated Health – Paramedicine (MIH-P) programs locally with Johnson County Ambulance Service as their MIH-P Medical Director and statewide as Iowa’s MIH-P Development Co-Chair. He works with Ceribell Point of Care EEG conducting innovative emergency medicine research.

Tiffany Vaughn, RN, BSN, has been an RN for almost 15 years, primarily in cardiac nursing. Following 7 years of bedside nursing she spent 5 years at a private cardiology practice working for a cardiologist specializing in Structural Heart Procedures. During 12 years in both inpatient and outpatient cardiac care, Tiffany saw first-hand how a patient's social determinants of health impacted their outcomes. Tiffany's more recent role as the HFC Nurse Navigator has given her the opportunity to do what she loves-advocate. She strives to advocate for her patients to get the best available care possible. As Nurse Navigator her primary goal is to "find" patients that are hospitalized with Heart Failure and provide 7 day follow up at the HFC. While seeing these patients she tries to get to the root cause of why they have multiple HF hospitalizations. For patients with significant social determinants of health, she recommends the HFC MIH-CP program. She then sets up Telehealth visits coordinating the Paramedic and the Nurse Practitioner at whichever HFC the patient lives closest to.

Todd Barton has served as Mayor of Crawfordsville since January 1, 2012 and is currently serving his fourth term.

Mayor Barton is a native of Crawfordsville and Montgomery County and served the community for 26 years as a fire fighter/paramedic with the Crawfordsville Fire Department.

He graduated with honors from Wabash College in 2000 with a B.A., majoring in political science. He earned his degree in three years by attending the liberal arts school full-time, working full-time and raising a family.

As mayor he has led an ambitious and successful effort to restore financial stability, rebuild infrastructure, improve quality of life and modernize economic development efforts through business retention, attraction and workforce development.

Under his leadership several innovative programs have been developed in Crawfordsville that have received statewide, national and international attention. These include Mobile Integrated Health, Project Swaddle, Project Bundle, Quick Response Team to address addiction and mental health, Creation of a Workforce Alliance, the Montgomery County Early Childhood Coalition, a monthly community forum, and a weekly podcast.

He and his wife Jamie, a nurse practitioner in Crawfordsville, have three grown children

The Olathe Fire Department Mobile Integrated Healthcare team connects patients with the right care in the right place at the right time. We have two teams, each made up of a firefighter paramedic and nurse practitioner. Our program is a three-way partnership with our local hospital, our federally qualified health center, and our fire department. We provide primary care services to Olathe community members at no cost to them, wherever they need it. These services also include referrals to behavioral health services and substance use treatment.
